[目的]观察血塞通注射液对呼吸衰竭机械通气患者并发应激性溃疡的影响。[方法]78例因呼吸衰竭而气管插管机械通气患者随机分成对照组(n=42)和治疗组(n=36)。对照组给予呼吸衰竭常规治疗,治疗组在此基础上予血塞通注射液8ml稀释后静脉滴注,1次/d,疗程为7d。采用Griess法测定患者血清NO浓度;采用硫代巴比妥酸比色法测定血清MDA水平;采用黄嘌呤氧化酶法测定血清SOD浓度,并观察治疗中及治疗结束后应激性溃疡发生率。[结果]最终有71例患者完成了研究(治疗组33例,对照组38例)。治疗组患者应激性溃疡发生率为21.21%,对照组患者发生率为47.37%,治疗组发生率较对照组明显下降(P<0.05)。治疗前两患者血清NO、MDA及SOD水平均无统计学差异(P>0.05),治疗后治疗组血清MDA水平较对照组明显下降(P<0.05),而血清NO、SOD水平则明显上升(P<0.01,P<0.05)。[结论]血塞通注射液能有效降低呼吸衰竭机械通气患者应激性溃疡发生率,其可能通过抗氧化,清除氧自由基的作用。
[Objective] To observe the effect of Xuesaitong injection on stress ulcer complicated with respiratory failure in patients with mechanical ventilation. [Methods] 78 patients with mechanical ventilation due to respiratory failure and intubation were randomly divided into control group (n = 42) and treatment group (n = 36). The control group was given routine treatment of respiratory failure. On the basis of this, the treatment group was given intravenous drip of Xuesaitong injection 8ml diluted once a day for 7 days. Serum NO concentration was measured by Griess method. Serum MDA level was measured by thiobarbituric acid colorimetric assay. Serum SOD concentration was measured by xanthine oxidase method. The incidence of stress ulcer during and after treatment was observed. [Results] Finally, 71 patients completed the study (33 in the treatment group and 38 in the control group). The incidence of stress ulcer was 21.21% in the treatment group and 47.37% in the control group. The incidence of the treatment group was significantly lower than that of the control group (P <0.05). The levels of serum NO, MDA and SOD in the two groups before treatment were not significantly different (P> 0.05). After treatment, serum MDA level in the treatment group was significantly lower than that in the control group (P <0.05), while serum NO and SOD levels were significantly increased P <0.01, P <0.05). [Conclusion] Xuesaitong injection can effectively reduce the incidence of stress ulcer in patients with respiratory failure and mechanical ventilation, which may be through the role of antioxidant and scavenging oxygen free radicals.
