血清游离 DNA 对宫颈癌患者的早期诊断和预后判断价值研究

目的:探讨血清游离 DNA 对宫颈癌患者的早期诊断和预后判断价值。方法选取2012年5月至2014年2月我院收治的宫颈癌患者45例,并随机选择同期在我院体检的健康女性50名,采用电化学发光分析仪检测血清游离 DNA(cf-DNA)、糖类抗原125(CA125)、糖类抗原19-9(CA19-9)、鳞状细胞癌抗原(SCC)、β人绒毛膜促性腺激素(β-HCG)浓度。结果与健康对照组相比,宫颈癌组患者血清 cf-DNA[(178.8±43.4)、(1354.7±267.2)μg/ L]、SCC[(0.7±0.3)、(1.4 ± 0.4)μg/ L]、β-HCG [(1.3 ± 0.4)、(5.2 ± 0.6)kU/ L ]水平明显升高(t 值分别为30.690、9.709、37.614,P 均<0.001);宫颈癌临床分期为Ⅱ、Ⅲ期的患者血清 cf-DNA 水平明显高于Ⅰ期患者[(2538.8±775.3)、(1244.7±253.6)μg/ L,t =8.191、P <0.001];血清 cf-DNA、CA125、CA19-9、SCC、β-HCG 五种诊断指标 ROC 曲线下面积分别为0.9516、0.6382、0.6144、0.8057、0.7531。cf-DNA 敏感度为75.56%、特异度为99.13%、准确度为93.36%;CA125敏感度为28.32%、特异度为93.24%、准确度为64.26%;CA19-9敏感度为19.75%、特异度为94.47%、准确度为67.28%;SCC 敏感度为38.87%、特异度为88.53%、准确度为80.16%;β-HCG 敏感度为43.28%、特异度为86.21%、准确度为77.83%。结论血清 cf-DNA 对宫颈癌患者的早期诊断和预后判断有较高的敏感度、特异度和准确度,值得临床推广。
