目的 :评估骨髓活检在急性白血病继发骨髓纤维化时的诊断价值。方法 :对比分析 34例急性白血病继发骨髓纤维化的骨髓活检切片与涂片增生度 ,动态观察 10例急性粒细胞白血病化疗过程中网硬蛋白纤维含量变化。结果 :网硬蛋白纤维可致白血病骨髓涂片假性增生减低 ,骨髓活检切片能起修正作用。伴骨髓纤维化的早期白血病骨髓切片较涂片更早作出诊断。网硬蛋白纤维含量的变化与白血病化疗疗效有关。结论 :骨髓活检切片对涂片起补充修正作用 ,诊断急性白血病须把二者结合起来分析。
Purpose : To evaluate the diagnostic value of bone marrow biopsy in the diagnosis of myeloid fibrosis secondary to acute leukemia. METHODS: The bone marrow biopsy specimens and smear hypertrophy of 34 patients with acute leukemia secondary to bone marrow fibrosis were compared and analyzed. The changes of reticulin fiber content in 10 acute myeloid leukemia patients during chemotherapy were dynamically observed. RESULTS: Mesh fibrin fibers could cause pseudohyperplasia of leukemia bone marrow smears, and bone marrow biopsy slices could play a corrective role. Early leukemia bone marrow sections with myelofibrosis were diagnosed earlier than smears. The change in reticulin content in fibrin is related to the efficacy of leukemia chemotherapy. Conclusion : Bone marrow biopsies have a supplementary correction effect on smears. To diagnose acute leukemia, the two must be combined and analyzed.
