目的:观察大黄芒硝外敷对剖宫产术后子宫切口血肿的疗效。方法:将行子宫下段横切口剖宫产术术后发生子宫切口处稳定型血肿者共16例,随机分为2组,各8例。中药组以生大黄、芒硝粉碎后混匀,以适量食醋调和,包好后敷于下腹部血肿处;理疗组予TDP治疗器照射下腹部。观察3月。结果:2组体温恢复情况比较,差异有统计学意义(P<0.05)。治疗后3天,中药组白细胞、中性粒细胞水平均较治疗前明显下降(P<0.05,P<0.01);理疗组中性粒细胞水平亦明显下降(P<0.01);治疗后1周,中药组白细胞、中性粒细胞水平基本降至正常生理范围,与理疗组比较,差异有统计学意义(P<0.05)。中药组血肿吸收速度明显快于理疗组,血肿在治疗后1周、治疗后1月、治疗后2月至治疗后3月,与理疗组比较,差异有统计学意义(P<0.05)。结论:剖宫产术后子宫切口血肿者,采用大黄、芒硝以食醋调和外敷,能明显缩短血肿吸收时间,减少感染率。
Objective: To observe the curative effect of rhubarb mirabilite on uterine incision hematoma after cesarean section. Methods: A total of 16 cases of stable hematoma occurred at the uterine incision after cesarean section underwent transverse incision in the lower uterine segment were randomly divided into 2 groups (n = 8). The Chinese medicine group rhubarb, Glauber’s salt mixture after crushing, with appropriate amount of vinegar to reconcile, wrapped in Fu under the abdominal hematoma; Physiotherapy group to the TDP irradiation under the abdomen. Observe March. Results: There was significant difference between the two groups in the recovery of body temperature (P <0.05). Three days after treatment, the levels of leukocytes and neutrophils in Chinese medicine group were significantly lower than those before treatment (P <0.05, P <0.01), while the levels of neutrophils in physiotherapy group were also significantly decreased (P <0.01) , While the level of leukocyte and neutrophil in the traditional Chinese medicine group decreased to the normal physiological range. The difference was statistically significant (P <0.05) compared with the physical therapy group. The rate of hematoma absorption in TCM group was significantly faster than that in physiotherapy group. The hematoma in 1 week after treatment, 1 month after treatment, 2 months after treatment and 3 months after treatment showed significant difference compared with physiotherapy group (P <0.05). Conclusion: Cesarean section after uterine incision hematoma, the use of rhubarb, Glauber’s salt with vinegar and external application, can significantly reduce the time of hematoma absorption and reduce the infection rate.
