目的探讨慢性乙肝病毒(hepatitis B virus,HBV)感染者HBeAg阳性率和丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)水平。方法对居民健康体检发现的HBsAg阳性者用酶联免疫吸附法(ELISA)检测乙肝病毒血清标志物(HBV-M),用试剂盒(速率法)检测丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)。结果共检测HBsAg阳性者3 721例,其中男性2 008例,女性1 713例,平均(44.03±8.54)岁。HBeAg阳性605例,阳性率为16.26%,其中男性和女性阳性率分别为18.13%,14.07%,男女差异有统计学意义(P<0.01)。HBeAg阳性组的ALT、AST平均值均>HBeAg阴性组(P<0.01)。HBeAg阳性组ALT、AST>80u/L的异常率均高于阴性组,差异有统计学意义(P<0.01)。男性ALT、AST平均值和异常率(>40u/L)均大于女性,差异有统计学意义(P<0.01)。感染者ALT/AST为1.07,其中,HBeAg阳性与阴性组分别为1.28、1.04;10~40岁年龄组均>1,<10岁组和50岁以上年龄组均<1。结论慢性HBV感染者以HBeAg阴性为主,HBeAg阳性率存在性别差异,HBeAg阳性者和大年龄的感染者肝功能损害更明显。
Objective To investigate the positive rate of HBeAg and the levels of alanine aminotransferase (ALT) and aspartate aminotransferase (AST) in patients with chronic hepatitis B virus (HBV) infection. Methods Serum markers of HBV (HBV-M) were detected by enzyme-linked immunosorbent assay (ELISA) in HBsAg-positive patients detected by health check-ups of residents. The alanine aminotransferase (ALT) Aspartate aminotransferase (AST). Results A total of 3721 HBsAg positive cases were detected, including 2 008 males and 1 713 females, with an average of (44.03 ± 8.54) years. The positive rate of HBeAg in 605 cases was 16.26%. The positive rates of male and female were 18.13% and 14.07%, respectively. The difference was statistically significant (P <0.01). The average values of ALT and AST in HBeAg positive group were> HBeAg negative (P <0.01). The abnormal rates of ALT and AST> 80u / L in HBeAg positive group were higher than those in negative group, the difference was statistically significant (P <0.01). The mean and abnormal rates of ALT and AST in males were higher than those in females (P <0.01). The ALT / AST of infected persons was 1.07, of which, HBeAg positive and negative groups were 1.28 and 1.04, respectively. All the patients aged 10-40 years old were> 1, <10 years old and those over 50 years old <1. Conclusions Chronic HBV infection is predominantly HBeAg-negative with gender differences in HBeAg positive rates. Liver damage is more evident in HBeAg-positive and older patients.
