目的:探讨血清中乙型肝炎病毒(HBV)共价闭合环状DNA(cccDNA)全序列的PCR检测与HBV复制及肝细胞损伤的临床相关性。方法:用长链PCR方法对乙型肝炎患者血清进行HBV cccDNA全序列直接扩增,同时检测血清中乙型肝炎病毒前S1(PreS1)抗原和丙氨酸氨基转移酶(ALT)活性。结果:cccDNA和PreS1抗原在乙型肝炎病毒e抗原(HBeAg)阳性血清中检出率分别为43.94%(29/66)和83.33%(55/66),在HBeAg阴性血清中的检出率分别为13.33%(4/30)和30.00%(9/30);cccDNA阳性血清的PreS1抗原阳性率和ALT活性升高分别为90.91%(30/33)和93.94%(31/33),cccDNA阴性血清的PreS1抗原阳性率和ALT活性升高分别为53.97%(34/63)和41.27%(26/63)。结论:血清中cccDNA的检出与PreS1抗原、HBeAg的表达以及肝细胞损伤呈高度相关性。
Objective: To investigate the clinical significance of PCR detection of hepatitis C virus (HBV) covalently linked circular DNA (cccDNA) sequence in HBV replication and hepatocellular injury. Methods: The HBV cccDNA sequence was directly amplified from the serum of patients with chronic hepatitis B by polymerase chain reaction (PCR). PreS1 antigen and alanine aminotransferase (ALT) activity in serum were also detected. Results: The detection rates of cccDNA and PreS1 antigen in HBeAg-positive sera were 43.94% (29/66) and 83.33% (55/66), respectively, and the detection rates in HBeAg-negative sera were The positive rate of PreS1 antigen and the increase of ALT activity of cccDNA positive sera were 90.91% (30/33) and 93.94% (31/33), respectively. The positive rates of cccDNA negative sera were 13.33% (4/30) and 30.00% (9/30) The serum PreS1 antigen positive rate and ALT activity increased 53.97% (34/63) and 41.27% (26/63) respectively. Conclusion: The detection of cccDNA in serum is highly correlated with PreS1 antigen, HBeAg expression and hepatocellular injury.
