目的探讨在B超引导下125I放射性粒子植入治疗胸壁及颈部转移瘤方法及疗效。方法术前根据肿瘤大小、粒子剂量、放射有效范围等利用TPS系统计算出粒子分布及最佳数量,术中在超声引导下经皮穿刺将粒子按计划植入预定部位,术后CT薄扫粒子植入部位及检查有无漏植。结果8例胸壁转移瘤及12例颈部转移瘤患者放射性粒子均成功植入预定部位,所有患者未出现术后并发症,术后9例患者肿瘤完全缓解(CR),6例患者肿瘤部分缓解(PR)缩小>50%,5例肿瘤部分缩小(缩小<50%),总有效率(CR+PR)75%(15/20)。2个月、半年、1年随访中,无复发病例。结论放射性粒子永久植入人体治疗肿瘤疗效确切,B超引导下粒子植入具有实时监视、定位准确、方法简单、无放射损伤等优点。
Objective To investigate the method and efficacy of 125I radioactive seed implantation in the treatment of chest wall and neck metastases under B-guided ultrasound. Methods TPS system was used to calculate the particle distribution and the optimal number according to tumor size, dose and radiation effective range. The intraoperative percutaneous puncture under the guidance of ultrasound guided the implantation of the particles into the planned site. The postoperative CT thinning particles Implantation site and check for missing vegetation. Results The radioactive particles were successfully implanted in 8 cases of chest wall metastases and 12 cases of cervical metastases. No postoperative complications were found in all the cases. Complete remission (CR) occurred in 9 cases and partial remission in 6 cases (PR) was reduced by> 50%. The tumor size was partially reduced (<50%) and the total effective rate (CR + PR) was 75% (15/20) 2 months, half a year, 1 year follow-up, no recurrence. Conclusion The radioactive particles are permanently implanted into the human body to treat the tumor. The B-guided particle implantation has the advantages of real-time monitoring, accurate positioning, simple method and no radiation injury.
