目的总结分析血糖控制情况对肺结核合并糖尿病患者化学治疗的疗效影响。方法选择2008年10月1日至2011年10月1日期间收治的100例肺结核患者为研究对象,其中52例血糖水平异常的肺结核患者为观察组(再按血糖控制达标和不达标分为观察1组32例和观察2组20例),48例血糖正常的肺结核患者为对照组,研究组在治疗的同时给予胰岛素为主的血糖控制治疗,观察三组治疗疗效差异。结果观察组X线胸片表现中单肺(61.54%)、多叶段(82.69%)、干酪空洞(57.69%)明显高于对照组;观察组痰菌阳性率(57.69%)明显高于对照组(25.00%),差异具有统计学意义(P<0.01)。治疗3个月末血糖控制较好的观察1组痰菌组转阴率66.67%明显高于观察2组的16.67%,治疗12个月末观察1组痰菌组转阴率88.89%明显高于观察2组的41.67%,差异均有统计学意义(P均<0.05);观察1组临床痊愈率(59.38%)明显高于观察2组(25.00%),差异具有统计学意义(P<0.05)。结论肺结核患者的血糖水平与化学治疗疗效有密切关系,要严格控制血糖,以期争取更佳疗效。
Objective To summarize and analyze the effect of glycemic control on the curative effect of chemotherapy in patients with pulmonary tuberculosis complicated with diabetes mellitus. Methods A total of 100 patients with pulmonary tuberculosis who were treated between October 1, 2008 and October 1, 2011 were enrolled in this study. Among them, 52 patients with abnormal blood glucose level were treated as observation group 32 cases in group 1 and 20 cases in observation group 2). 48 cases of patients with normal blood sugar were selected as control group. The study group was given insulin-based glycemic control treatment at the same time, and the differences between the three groups were observed. Results In the observation group, the single lung (61.54%), multilobate (82.69%) and cheese cavity (57.69%) were significantly higher in the observation group than in the control group. The positive rate of sputum in the observation group was significantly higher than that of the control Group (25.00%), the difference was statistically significant (P <0.01). 3 months after treatment, blood glucose control better observation group 1 sputum bacterial group was 66.67% negative conversion rate was significantly higher than the observation group 2 16.67%, 12 months after treatment to observe a group of sputum bacteria negative conversion rate 88.89% was significantly higher than the observation 2 (P <0.05). The clinical cure rate (59.38%) in observation group 1 was significantly higher than that in observation group 2 (25.00%), the difference was statistically significant (P <0.05). Conclusion The blood sugar level of patients with tuberculosis is closely related to the curative effect of chemotherapy. The blood sugar should be strictly controlled so as to achieve better curative effect.
