目的探究血清半乳凝素-3(GAL-3)、血清糖类抗原153(CA153)和肿瘤特异生长因子(TSGF)联合乳腺钼靶X线征象用于乳腺癌早期诊断的临床价值。方法选取2015年1月至2016年10月间深圳市龙岗区第二人民医院收治的62例乳腺癌患者、53例乳腺良性疾病患者和48例健康体检者,分别设为恶性组、良性组和对照组,分别采用酶联免疫吸附测定法、电化学发光免疫法和生化比色法测定三组受检者的GAL-3、CA153和TSGF水平,并拍摄乳腺钼靶X线,对比GAL-3、CA153和TSGF联合检测率及GAL-3、CA153、TSGF结合乳腺钼靶X线征象检测率。结果恶性组患者的GAL-3、CA153和TSGF水平均显著高于良性组和对照组,差异均有统计学意义(均P<0.05);良性组患者的GAL-3和TSGF水平显著高于对照组,差异均有统计学意义(均P<0.05),但CA153水平与对照组相比,差异无统计学意义(P>0.05)。单项检测结果显示,敏感性乳腺钼靶X线>TSGF>GAL-3>CA153,各组间对比,差异均有统计学意义(均P<0.05);特异性乳腺钼靶X线>CA153>GAL-3>TSGF,但组间对比,差异均无统计学意义(均P>0.05)。联合检测结果显示,GAL-3+CA153+TSGF+乳腺钼靶X线检测、GAL-3+CA153+TSGF检测与单纯采用乳腺钼靶X线检测的敏感性对比,差异均无统计学意义(均P>0.05),但均显著高于单项检测。GAL-3+CA153+TSGF检测与单项检测的特异性对比,差异均无统计学意义(均P>0.05),但GAL-3+CA153+TSGF+乳腺钼靶X线检测的特异性均显著高于其他检测方案,差异均有统计学意义(均P<0.05)。结论 GAL-3、CA153、TSGF结合乳腺钼靶X线征象用于乳腺癌的早期诊断有着较好的敏感性及特异性,相较于单项检测有更好的应用价值,值得临床推广。
Objective To investigate the clinical value of serum Galectin-3 (GAL-3), serum carbohydrate antigen 153 (CA153) and tumor-specific growth factor (TSGF) combined with mammography in the early diagnosis of breast cancer. Methods From January 2015 to October 2016, 62 cases of breast cancer, 53 cases of benign breast diseases and 48 cases of healthy subjects admitted to the Second People’s Hospital of Longgang District of Shenzhen City were selected as malignant group, benign group and benign group In the control group, the levels of GAL-3, CA153 and TSGF were measured by enzyme-linked immunosorbent assay (ELISA), chemiluminescence immunoassay and biochemical colorimetry, respectively. Mammograms of mammograms were taken and compared with those of GAL-3 , The combined detection rate of CA153 and TSGF, and the detection rate of mammographic X-ray signs of GAL-3, CA153 and TSGF. Results The levels of GAL-3, CA153 and TSGF in malignant group were significantly higher than those in benign group and control group (all P <0.05). The levels of GAL-3 and TSGF in benign group were significantly higher than those in control group (P <0.05). However, there was no significant difference between CA153 and control group (P> 0.05). The results of single test showed that the sensitivity of mammography in X-ray> TSGF> GAL-3> CA153 was statistically significant (all P <0.05); the specific mammography X-ray> CA153> -3> TSGF, but there was no significant difference between the two groups (all P> 0.05). The results of joint detection showed that there was no significant difference in the sensitivity of GAL-3 + CA153 + TSGF + mammography and GAL-3 + CA153 + TSGF between the two groups > 0.05), but were significantly higher than the single test. The specificity of GAL-3 + CA153 + TSGF detection and single detection was not significantly different (all P> 0.05), but the specificity of GAL-3 + CA153 + TSGF + mammography was significantly higher than that of GAL- Other test programs, the differences were statistically significant (P <0.05). Conclusion The combination of GAL-3, CA153, TSGF and mammography in the early diagnosis of breast cancer has good sensitivity and specificity, which is worthy of clinical promotion compared with the single detection.
