[摘要]目的:探讨不同前牙转矩DamonQ自锁托槽的临床使用方法。方法:用不同转矩DamonQ自锁托槽技术矫治各类病例80余例,其中安氏I类43例,安氏Ⅱ类27例,安氏Ⅲ类15例。结果:治疗后所有患者上下牙列排列整齐,覆牙合、覆盖正常,磨牙尖牙关系中性;通过牙根移动,改变切牙倾度,软组织侧貌改善明显;减少了矫治后期需要对切牙的转矩控制所消耗的时间。结论:选择合适转矩的DamonQ自锁托槽是一种简便、有效的控根方法,使切牙美观和功能得到进一步改善。

Abstract: Objective The aim of this work is to explore the clinical applications of different methods of anterior torque DamonQ self-ligating bracket. Methods More than 80 cases, including 43 cases of Angle Class I,27 cases of Angle Class Ⅱ,15 cases of Class Ⅲ,were treated with different self-ligating bracket torque DamonQ technology. Results After treatment, all patients showed neat upper and lower dentition,normal laminated cover,and neutral molar canine relationship.This indicates the significant improvement of the incisor inclination and soft tissue profile through the movement of tooth roots,thereby reducing the time needed for the post-treatment of incisor torque control. Conclusion Selection of the appropriate torque DamonQ self-ligating bracket is a simple and effective method for controlling tooth roots, further enhancing incisor appearance and functionality.

直丝弓托槽包括MBT矫治器、主动或被动自锁托槽都是统一预置转矩的托槽[1]。弓丝与托槽槽沟存在余隙角,如果对切牙转矩控制不足,则需要在后期精细调整阶段使用转矩辅弓[2]或弓丝上弯制转矩进行转矩控制,增加了医生的椅旁操作时间和患者复诊次数和疗程。托槽的转矩角及余隙角是影响牙齿转矩表达的因素[3]。DamonQ自锁托槽根据临床需要提供不同转矩数值的自锁托槽(见表1)。根据治疗前牙齿的位置和治疗后牙齿需要达到的理想位置决定选择切牙和尖牙托槽转矩,并结合对于余隙角的考虑,在第一根细方形弓丝时就开始控根移动,减少了后期需要做控根治疗时弯制弓丝的时间,并且在矫治过程中维持了前牙的倾斜度,有助于避免不必要的牙根移动,同时减少了椅旁工作时间和复诊的次数,实现了轻力矫治,改进了牙根的位置。
1 资料和方法
采用不同转矩DamonQ自锁托槽技术矫治各类病例80余例,其中安氏I类43例,安氏Ⅱ类27例,安氏Ⅲ类15例。弓丝顺序为:0.014"(1"=2.54cm)含铜Ni-Ti丝;0.014"×0.025"含铜NiTi丝;0.019"×0.025"热激活Ni-Ti丝;0.019"×0.025"不锈钢方丝。
2 结果
采用不同转矩DamonQ自锁托槽技术能够矫治所有错牙合畸形病例,治疗后所有患者上下牙列排列整齐,覆牙合、覆盖正常,尖牙关系中性,磨牙咬合关系良好;通过牙根移动,改变切牙倾度,软组织侧貌改善明显;减少了矫治后期需要对切牙的转矩控制所消耗的时间。
3 典型病例
某女,21岁,上前牙间隙要求治疗。临床检查:第一磨牙远中关系,覆盖5mm,Ⅱ度深覆牙合,14、15、25缺失。矫治设计:关闭15、25间隙,14修复治疗。考虑到上切牙倾斜度正常,为防止矫治后上切牙过于直立,上切牙选用高转矩DamonQ自锁托槽,下牙标准转矩。矫治后覆牙合、覆盖正常,磨牙完全远中关系。U1-NA减小了1.1°,U1-SN只减小了1.9°,头颅侧位重叠图显示牙冠和牙根都远中移动,可见高转矩的切牙托槽对牙轴的控制很好(如图1~3,见表2)。
