专业学位硕士研究生和住院医师并轨培养模式下工作学习状态调查研究

目的:了解专业学位硕士研究生(下文简称“专硕”)和住院医师并轨培养下工作学习状态,为住院医师规范化培训(下文简称“住培”)相关政策的制定提供参考。方法:2019年7月至8月对北京市20家住培基地476名并轨培养的专硕和住院医师进行横断面调查。调查内容包括性别、年龄、所属高校等基线资料,培训基地、培训年限等培训信息,工作时间、值班次数、休假等工作学习状态,对基地管理、福利待遇、价值实现等的满意度,学习压力、工作需求、归属感等身心状况等。结果:并轨状态下专硕和住院医师工作时间长[平均每周工作时间小于40小时者仅占3.2%(15/476)],值班次数多[平均每月值班次数大于3次者占69.7%(332/476)],工作学习压力大(平均74.06)。85.7%(408/476)的学员同意或基本同意导师通过各种途径为其提供了增加新经验和成长的机会,76.5%(364/476)的学员同意或基本同意培训对其实际能力提升有较大帮助;专硕和住院医师认可并轨模式,如有机会重新选择,78.2%(372/476)的学员仍会选择该模式,但普遍认为薪酬待遇相对较低,社会保险、基地福利和休假安排不能满足需求。结论:并轨培养的专硕和住院医师对并轨模式满意度较高,但是学员自觉工作强度和压力较大,对薪酬待遇福利等的满意度不高,其心理压力、情绪反应和进步需求需要得到关注。“,”Objective:To better understand the working and learning status of professional degree postgraduates and residents under the combined training mode, so as to provide basis and reference for the formulation of relevant policies of the state and Beijing.Methods:A cross-sectional survey was conducted among 476 trainees in 20 training bases of Beijing from July to August 2019. The survey contents include: baseline information such as gender and age, training information such as university, training base, training period, working and learning state information such as working time, duty times, and leave regulations, satisfaction with base management, welfare treatment and value realization, physical and mental conditions such as learning pressure, work demand and ownership feeling. Statistical description method was used for analysis.Results:Under the combined training mode, the trainees have long working hours[only 3.2% (15/476) worked less than 40 hours per week], many times on duty[69.7% (332 / 476) were on duty more than three times a month], and the average value of working and learning pressure has reached 74.06; 85.7% (408 / 476) of the trainees agreed or basically agreed that the tutor provided them with the opportunity to increase their new experience and growth through various ways, and 76.5% (364 / 476) of the trainees agreed or basically agreed that the training would be helpful to their actual ability improvement; the mode was approved by master degree and resident doctors, and 78.2% (372 / 476) would still choose the mode if they had the opportunity to choose again, However, it is generally believed that the salary is relatively low, and social insurance, base welfare and vacation arrangement can not meet the demand.Conclusions:The trainees are satisfied with the combined training mode, but they are more aware of their work intensity and pressure, less satisfied with their salary and welfare; their psychological pressure, emotional response and progress requirement need to be paid attention to.
