目的探讨双源CT诊断胸痛三联征病因的临床价值。方法 50例患者作双源CT回顾性心电门控成像,采用多平面重组(MPR)、最大密度投影(MIP)、曲面重组(CPR)、及容积再现(VR)技术显示肺动脉、胸主动脉及冠状动脉,并评价图像质量是否满足临床诊断需要。结果所有患者均可清晰显示冠状动脉主干及主要分支、肺动脉及其分支、胸主动脉及胸部其它结构。检出冠状动脉狭窄≥50%者12例,LAD闭塞1例,肺动脉栓塞10例,主动脉夹层8例,壁间血肿2例,动脉导管未闭导管破例1例,气胸2例,动脉瘤2例,12例未见异常。结论双源CT能够一次扫描完成肺动脉、胸主动脉和冠状动脉的检查,图像质量可以满足临床诊断需要,在急性胸痛患者病因诊断中具有很高的临床应用价值。
Objective To investigate the clinical value of dual-source CT in the diagnosis of chest triad symptom. Methods Fifty patients underwent dual-source CT retrospective ECG-gated imaging. The pulmonary artery and thoracic aorta were visualized by multiplanar reconstruction (MPR), maximum density projection (MIP), surface reconstruction (CPR) And coronary artery, and evaluate the image quality to meet the needs of clinical diagnosis. Results All patients were able to clearly show the main coronary artery and the main branch, pulmonary artery and its branches, thoracic aorta and other structures. 12 cases with coronary stenosis≥50%, LAD occlusion in 1 case, pulmonary embolism in 10 cases, aortic dissection in 8 cases, intermural hematoma in 2 cases, patent ductus arteriosus in 1 case, pneumothorax in 2 cases, aneurysm 2 Cases, 12 patients without exception. Conclusion Dual-source CT scan can complete the examination of pulmonary artery, thoracic aorta and coronary artery in one scan. The image quality can meet the needs of clinical diagnosis, and has high clinical value in the diagnosis of acute chest pain.
