目的对比增强扫描前后正常肝脏、脾脏及肾脏ADC值的变化,探讨肝胆特异性对比剂钆塞酸二钠(Gd-EOB-DTPA)对腹部DWI的影响。资料与方法 21例志愿者于静脉注射Gd-EOB-DTPA前及注射20 min后分别行T1WI肝脏容积加速快速成像序列和扩散加权成像扫描(b值取0、800 s/mm2),分别测量并比较增强扫描前后T1WI图像的信噪比(SNR)及正常肝脏、脾脏及肾脏的表观扩散系数(ADC)值的变化。结果静脉注射Gd-EOB-DTPA 20 min后肝脏、肾脏的SNR分别为163.41±56.24、67.24±21.02,显著高于增强扫描前的101.33±37.58、42.70±14.72(t=5.797、6.247,P<0.01);增强扫描后脾脏的SNR为58.80±15.56,略高于增强前的51.95±13.03(t=2.150,P<0.05);增强扫描后正常肝脏和肾脏的ADC值分别为(1.27±0.23)×10-3 mm/s、(1.99±0.27)×10-3 mm/s,较增强扫描前的(1.34±0.25)×10-3 mm/s、(2.10±0.29)×10-3 mm/s降低(t=2.189,P<0.05;t=3.195,P<0.01);增强扫描前后脾脏的ADC值分别为(0.84±0.22)×10-3 mm/s和(0.78±0.25)×10-3 mm/s,差异无统计学意义(t=1.625,P>0.05)。结论静脉注射Gd-EOB-DTPA 20 min后T1WI图像正常肝脏、脾脏及肾脏的SNR均高于增强扫描前,有利于病变的显示及检出;增强扫描后肝脏及肾脏的ADC值降低,而脾脏的ADC值较增强扫描前无显著变化,静脉注射Gd-EOB-DTPA会影响正常肝脏及肾脏ADC值的测量。
Objective To compare the ADC value of normal liver, spleen and kidney before and after contrast-enhanced scanning and investigate the effect of hepatobiliary-specific contrast agent Gd-EOB-DTPA on abdominal DWI. Materials and Methods Twenty-one volunteers underwent T1WI hepatic volume-accelerated rapid imaging and diffusion-weighted imaging before and after injection of Gd-EOB-DTPA (b value: 0, 800 s / mm2) The signal-to-noise ratio (SNR) of T1WI images and the apparent diffusion coefficient (ADC) value of normal liver, spleen and kidney before and after contrast enhancement were compared. Results The liver and kidney SNRs of Gd-EOB-DTPA after intravenous injection for 20 min were 163.41 ± 56.24 and 67.24 ± 21.02, respectively, which were significantly higher than those before Gd-EOB-DTPA 101.33 ± 37.58,42.70 ± 14.72 (t = 5.797,6.247, P <0.01 ). The SNR of the spleen after enhanced scan was 58.80 ± 15.56, slightly higher than 51.95 ± 13.03 before enhancement (t = 2.150, P <0.05). The ADC values of the normal liver and kidney after enhanced scan were (1.27 ± 0.23) × (1.34 ± 0.25) × 10-3 mm / s, (2.10 ± 0.29) × 10-3 mm / s, (0.84 ± 0.22) × 10-3 mm / s and (0.78 ± 0.25) × 10-3, respectively (t = 2.189, P <0.05; t = 3.195, P <0.01) mm / s, the difference was not statistically significant (t = 1.625, P> 0.05). Conclusion The SNR of normal liver, spleen and kidney in T1WI images after intravenous injection of Gd-EOB-DTPA for 20 min is higher than that before enhanced scanning, which is beneficial to the display and detection of lesions. After enhanced scanning, the ADC value of liver and kidney decreased while the spleen ADC value had no significant change before enhancement scan. Intravenous injection of Gd-EOB-DTPA affected the measurement of ADC value in normal liver and kidney.
