目的为了使住院医师规范化培训工作达到标准化与同质化,本研究旨在建立一个以柯氏模型(kirkpatrick model)为基础的住院医师规范化培训效果评价指标体系,以便于对住院医师规范化培训的效果进行全面掌控,为规培工作的改进提供依据。方法成立研究小组,运用柯氏模型构建住院医师规范化培训效果评价指标体系草案,形成调查问卷,按要求选取北京、四川、江西共10名专家,对专家进行德尔菲法咨询,采用SPSS 18.0统计软件对结果进行统计分析,采用层次分析法(analytical hierarchy process,AHP)及百分权重法确立指标体系各指标的权重。结果 2轮专家咨询问卷回收率为100%,表示全部专家积极性均非常高,所构建的指标体系共包含有4个一级指标,11个二级指标以及45个三级指标,得出各级各指标所占的权重,2轮协调系数的检验中,P<0.01,差异有统计学意义,2轮肯德尔和谐系数均大于0.2,且第2轮协调系数与第1轮协调系数比有所提高,表明经过2轮咨询,专家对指标评判的意见最终达到了统一。结论本研究选取的专家的积极性及协调程度均较高,指标客观、全面、有重点,指标体系实际操作性较强,可作为住院医师规范化培训效果的评价工具,同时也可以为医院其他方面的培训工作提供参考资料,为推进国家专科医师规范化培训打下基础。
Objective To standardize and homogenize the standardization of residency training, this study aims to establish a standardized index system for evaluation of residency training based on kirkpatrick model, in order to facilitate the effect of standardized training of residents Conduct overall control and provide the basis for the improvement of the regulatory training. Methods A research team was set up to construct a draft standard for evaluating the effectiveness of standardized training of residents by using the Korotkoff model to form a questionnaire. Ten experts from Beijing, Sichuan and Jiangxi were selected according to the requirements. Delphi experts were used to consult Delphi and SPSS 18.0 statistical software The results of statistical analysis, the use of analytical hierarchy process (AHP) and percentage weight method to establish the index system weight of each indicator. Results The questionnaire recovery rate of two expert consultation questionnaires was 100%, which indicated that all the experts had very high enthusiasm. The index system consisted of 4 first-level indicators, 11 second-level indicators and 45 third-level indicators. The weight of each index, two rounds of coordination coefficient test, P <0.01, the difference was statistically significant, two Kendall harmony coefficient is greater than 0.2, and the second round of coordination coefficient and the first round of coordination coefficient ratio Improve, indicating that after two rounds of consultation, experts finally reached a unified opinion on the evaluation of indicators. Conclusion The experts selected in this research have high motivation and coordination degree. The indicators are objective, comprehensive and focused, and the index system is practical and can be used as evaluation tools for standardized training of residents, and also can be used for other aspects of hospital Training to provide reference materials, to promote the national specialist lay a foundation for standardized training.
