目的观察自酸蚀粘接剂结合釉质窝沟封闭术预防儿童第一恒磨牙龋齿的疗效及安全性。方法选取在2011年4月至2011年6月在郸城县人民医院口腔科门诊就诊的6~8岁学生40例,牙齿160颗,随机分为自酸蚀粘接剂组(牙齿80颗),采用自酸蚀粘接剂结合釉质窝沟封闭术,磷酸酸蚀组(牙齿80颗),37%磷酸酸蚀剂及釉质窝沟封闭术。封闭后半年、1年、2年进行复查,观察两组对象封闭剂的保存情况和封闭牙的患龋率情况。结果所有研究对象均完成治疗并进行了随访,随访率100%。与磷酸酸蚀组比较,自酸蚀粘接剂组的手术时间明显缩短,二者差异有统计学意义(P<0.05);自酸蚀粘接剂组的2年封闭剂保存率高于磷酸酸蚀组,2年龋齿率低于磷酸酸蚀组,二者差异有统计学意义(P<0.05);两组均无严重并发症发生。结论自酸蚀粘接剂结合釉质窝沟封闭术预防儿童第一恒磨牙龋齿,既可简化操作步骤,缩短时间,同时还可以增加封闭材料的密合性及粘接性,从而提高材料保留率。
Objective To observe the efficacy and safety of self-etching adhesive combined with enamel and fissure sealant in preventing children’s first permanent molars tooth decay. Methods From April 2011 to June 2011, 40 patients aged 6 to 8 years and 160 teeth in the Department of Stomatology, Dancheng People’s Hospital were randomly divided into self-etching adhesive group (80 teeth) , Self-etching adhesives combined with enamel and fissure sealant, phosphate acid etching group (80 teeth), 37% phosphoric acid etching agent and enamel fissure occlusion. Six months after the closure, one year and two years were reviewed to observe the preservation of the two groups of sealers and dental caries prevalence of closed teeth. Results All the subjects completed the treatment and were followed up with 100% follow-up. Compared with the acid etching group, the operation time of the self-etching adhesive group was significantly shortened (P <0.05); the self-etching adhesive group’s 2-year blocking agent retention rate was higher than that of the phosphoric acid In the acid etching group, the tooth decay rate in 2 years was lower than that in phosphate acid etching group, the difference was statistically significant (P <0.05). No serious complication occurred in both groups. Conclusion The combination of self-etching adhesive and enamel fissure sealant in preventing the first permanent molar tooth decay in children can not only simplify the operation steps and shorten the time, but also increase the closeness and adhesiveness of the sealant to improve the material retention rate .
