目的探讨骨科无痛病房建设中的临床应用效果。方法给予患者按时镇痛,明确组织架构与职责,进行持续性的人员培训、教育与考核,制订及实施疼痛控制程序,落实疼痛教育,进行阶段总结与反馈。结果实施无痛病房管理模式后,医护在疼痛评估、疼痛一般知识、药物镇痛知识、疼痛知识的综合应用4个指标的得分均高于此模式建立前,差异有统计学意义(P<0.05)。实施无痛病房管理后,患者接受调查时、过去24 h、手术后所经历的最严重疼痛程度均在5分以下(0~10级评分);患者对疼痛治疗方式、疼痛照顾、止痛效果的满意度评分在4分以上(5级评分),患者满意度评分高于实施前,差异有统计学意义(P<0.05)。结论骨科无痛病房管理的实施提高了医护的疼痛管理水平,提高了患者的疼痛控制质量,使患者得到了优质的无痛服务。
Objective To investigate the clinical application of orthopedic painless ward. Methods Patients were given analgesia on time, a clear organizational structure and responsibilities, continuous personnel training, education and assessment, formulation and implementation of pain control procedures, implementation of pain education, stage summary and feedback. Results After the implementation of painless ward management model, the scores of four indicators of comprehensive evaluation of health care in pain assessment, general pain knowledge, pain medicine knowledge and pain knowledge were all higher than those before the establishment of the model (P <0.05 ). After the implementation of painless ward management, the most severe pain experienced by the patients during the past 24 h after the operation was under 5 points (score 0 to 10) when they were surveyed. The patients’ pain management, pain relief, analgesic effect Satisfaction score of 4 points or more (grade 5), patient satisfaction score higher than before the implementation, the difference was statistically significant (P <0.05). Conclusion The implementation of orthopedic painless ward management improved the level of pain management in health care and improved the quality of pain control in patients so that the patients received excellent painless services.
