甲氰脒胍致变态反应性休克 男,28岁。患十二指肠溃疡予每日3次肌注甲氰脒胍0.4g,第5次用药后,全身出现散在性荨麻疹,搔痒。未予重视,第6次给药后患者突出现头昏、面色苍白、皮肤湿冷,脉率104,血压6.67/4.0kPa,即予抗变态反应处理而渐恢复。(龙件明) 甲氰脒胍致精神症状 男,81岁。因溃疡病出血予甲氰脒胍0.4g加于10％葡萄糖20ml中静注,每8小时一次,第4次注药后约半小时患者出现狂躁、谵妄等精神症状,虽予多种镇静剂,效果均不理想,停用甲氰脒胍后数小时即明显减轻,一天后完全复常。此种反应据报道发生率为7.3/10万,可能是药物阻滞了中枢神经H_2受体所致。(罗冬华)
