目的:在介入诊疗时,利用左室压力曲线描记和左室造影的方法评价冠心病介入治疗(PCI)前后左心室功能的变化。方法:冠心病患者共225例,50例正常人作为对照,所有研究对象均经冠状动脉造影明确诊断,分别于PCI前后连续记录左心室与主动脉压力曲线并取右前斜位30°行左室造影,测量并计算左室压力曲线和左室造影所体现的各项心室功能的指标,包括射血分数、左室舒张末压、室腔硬度常数、等容舒张期压力下降时间常数,并绘制心室压力-容积环。结果:冠心病患者与对照组的各项心功能指标均差异有统计学意义(P<0·05)。正常组心室压力-容积环位于坐标轴左下方,随冠状动脉狭窄病变的加重,典型压力-容积环向右上方移位。PCI后各项心功能指标均有改善,但不能完全恢复正常。结论:作为常规检查的左室压力曲线描记和左室造影的方法,可以较敏感、客观、全面、有效地反映左心室收缩和舒张功能。它能定量直观地在术中反映多项心室功能指标,可用于评价PCI对心功能的影响。
OBJECTIVE: To assess the changes of left ventricular function before and after interventional therapy (PCI) by means of left ventricular pressure curve and left ventricular angiography during interventional therapy. Methods: A total of 225 patients with coronary heart disease and 50 normal controls were included in the study. All subjects were diagnosed by coronary angiography. Left ventricular and aortic pressure curves were recorded continuously before and after PCI. Left ventricular Angiography, measurement and calculation of left ventricular pressure curve and left ventricular angiography embodied various indicators of ventricular function, including ejection fraction, left ventricular end-diastolic pressure, ventricular hardness constant, isovolumic diastolic pressure drop time constant, and draw Ventricular pressure - volume ring. Results: There were significant differences in cardiac function between patients with coronary heart disease and controls (P <0.05). The normal group ventricular pressure - volume ring located in the lower left axis, with the severity of coronary artery stenosis, the typical pressure - volume ring to the upper right shift. Cardiac function after PCI improved, but not completely returned to normal. Conclusion: As a routine examination of left ventricular pressure curve and left ventricular imaging method can be more sensitive, objective, comprehensive and effective reflection of left ventricular systolic and diastolic function. It can quantitatively and intuitively reflect a number of ventricular function indexes during operation, and can be used to evaluate the influence of PCI on cardiac function.
