目的初步探讨实时动态血糖监测(RTˉCGM)系统的准确性与血糖及血糖波动关系。方法纳入50例糖尿病患者,均应用RTˉCGM系统连续监测3 d。计算指尖血糖(FSBG)与RTˉCGM探头值的匹配率,用Pearson相关分析及多元线性回归分析分析匹配率与血糖波动参数的相关性。计算不同时间点、不同FSBG点精度的FSBG与对应探头值的相对平均偏差(RAD)和ISO标准。结果匹配率与平均血糖波动幅度(MAGE)呈线性负相关。全天、餐前、餐后、夜间探头值与相应FSBG的RAD分别为1.21%、2.50%、3.75%、2.38%(P<0.05),ISO标准分别为93.90%、95.02%、91.33%、95.04%。血糖(BG)<4.5 mmol/L,4.5≤BG<7.8 mmol/L,7.8≤BG<11.1mmol/L,11.1mmol/L≤BG探头值与相应FSBG的RAD分别为13.80%、3.14%、5.10%、6.81%(P<0.01),ISO标准分别为78.12%、97.19%、92.41%、89.50%。结论RTˉCGM准确性与血糖及血糖波动有关,血糖波动越小,准确率越高;当低血糖或高血糖时,准确性均降低。
Objective To investigate the relationship between the accuracy of real-time dynamic glucose monitoring (RTˉCGM) system and the fluctuation of blood glucose and blood glucose. Methods Fifty patients with diabetes mellitus were enrolled. All patients were monitored by RT ˉ CGM system continuously for 3 days. The matching rate of fingertip blood glucose (FSBG) and RTˉCGM probe was calculated. The correlation between matching rate and blood glucose fluctuation parameters was analyzed by Pearson correlation analysis and multivariate linear regression analysis. Calculate the relative average deviation (RAD) and ISO standard of FSBG and corresponding probe values at different time points and different FSBG point precision. Results Matching rate and mean blood glucose fluctuation range (MAGE) showed a linear negative correlation. The RAD values for all-day, pre-meal, postprandial and nighttime probe values and corresponding FSBG were 1.21%, 2.50%, 3.75% and 2.38%, respectively. The ISO standards were 93.90%, 95.02%, 91.33% and 95.04 %. The blood glucose (BG) <4.5 mmol / L, 4.5≤BG <7.8 mmol / L, 7.8≤BG <11.1 mmol / L, 11.1 mmol / L≤BG and the corresponding FSBG were 13.80%, 3.14% and 5.10 % And 6.81% respectively (P <0.01). The ISO standards were 78.12%, 97.19%, 92.41% and 89.50% respectively. Conclusion The accuracy of RT ˉ CGM is related to the fluctuations of blood glucose and blood glucose. The smaller the fluctuation of blood glucose is, the higher the accuracy of RT CGM is. The accuracy of RT CGM is lower than that of hypoglycemia or hyperglycemia.
