目的监测某区部队急性呼吸道传染病病原活动水平和流行动态,了解急性呼吸道传染病的病原谱构成,为部队卫生防病工作提供科学依据。方法对某区疾控中心和基层医院采集的流感样(influenza-like illness,ILI)病例进行监测分析,并完成病原谱的构建。结果某区疾控中心与基层医院共接收流感样病例鼻咽拭子1 161份,PCR检出阳性312份,总阳性检出率26.87%。其中疾控中心接收鼻咽拭子样本570份,检出阳性145份,阳性检出率25.44%;样本采集量于1、2月达到高峰。阳性率在3月左右达到高峰,比ILI病例高峰出现时间延后1~2个月。基层医院接收样本591份,检出阳性167份,阳性检出率28.26%。样本采集量近3月左右达到高峰。阳性率在2月左右达到高峰。病原主要以乙型流感病毒和甲型流感病毒为主,腺病毒及其他病毒亚型为辅,其中甲型以H3和H1N1两种亚型为主。结论本研究初步描述了某区部队发热病例常见呼吸道病毒病原谱,分析了呼吸道病毒性感染的流行病学分布,将对部队呼吸道病毒性疾病的临床诊治、传染病防控提供基础资料和参考。
Objective To monitor the level of pathogens and the epidemic dynamics of acute respiratory infectious diseases in a certain area and to understand the composition of the pathogenic spectrum of acute respiratory infectious diseases and to provide a scientific basis for the military health and disease prevention. Methods The surveillance and analysis of influenza-like illness (ILI) collected in the CDC and grassroots hospitals in a certain district were completed and the pathogenic spectrum was constructed. Results A total of 1 161 nasopharyngeal swab samples were collected from the CDC and grassroots hospitals in a district. 312 positive samples were detected by PCR, and the total positive detection rate was 26.87%. The CDC received 570 samples of nasopharyngeal swabs, 145 were positive, the positive detection rate was 25.44%; sample collection peaked in January and February. The positive rate reached a peak in around March, which lagged from the peak of ILI cases by 1 to 2 months. The grassroots hospitals received 591 samples, of which 167 were positive and the positive rate was 28.26%. Sample collection peaked in around March. The positive rate peaked in around February. Pathogen mainly influenza B virus and influenza A virus, adenovirus and other virus supplemented by subtypes, of which type A H3 and H1N1 two subtypes. Conclusions This study initially described the common respiratory virus pathogens in fever cases in a certain area and analyzed the epidemiological distribution of respiratory viral infections, which will provide the basic information and references for the clinical diagnosis and treatment of infectious diseases of respiratory diseases.
