目的探讨机械通气应用于肺动脉高压伴呼吸衰竭患者对其血气分析以及血浆脑钠肽(BNP)水平的影响。方法 90例行机械通气治疗的肺动脉高压伴呼吸衰竭患者随机分为观察组和对照组,每组45例,观察组采用有创机械通气治疗,对照组采取无创机械通气治疗,比较两组患者治疗后的血气分析指标变化以及BNP水平变化。结果治疗后观察组BNP指标与对照组相比明显下降,治疗后两组血气分析指标以及肺动脉压均较治疗前明显改善,且观察组更加明显,差异均有统计学意义(P<0.05)。结论机械通气应用于肺动脉高压伴呼吸衰竭患者的治疗能够起到显著的效果,且不同的机械通气方式效果有差异,有创机械通气疗效优于无创机械通气治疗,二者各有优缺点,临床应用上应该综合考虑,科学选择。
Objective To investigate the effects of mechanical ventilation on blood gas analysis and plasma brain natriuretic peptide (BNP) levels in patients with pulmonary hypertension and respiratory failure. Methods Ninety patients with pulmonary hypertension and respiratory failure undergoing mechanical ventilation were randomly divided into observation group and control group, with 45 cases in each group. The observation group was treated with invasive mechanical ventilation while the control group was treated with noninvasive mechanical ventilation. The two groups were compared Blood gas analysis after the index changes and changes in BNP levels. Results After treatment, the BNP level in the observation group was significantly lower than that in the control group. The indexes of blood gas analysis and pulmonary arterial pressure in the observation group were significantly improved after treatment, and the observation group was more obvious with statistical significance (P <0.05). Conclusion The application of mechanical ventilation in pulmonary hypertension patients with respiratory failure can play a significant effect, and the effect of different mechanical ventilation are different, invasive mechanical ventilation is better than non-invasive mechanical ventilation treatment, both have their own advantages and disadvantages, clinical Application should be considered, the scientific choice.
