目的 研究c -met基因蛋白和细胞凋亡在胃黏膜病变演进的表达及关系 ,探讨c -met表达对胃癌 (GC)预后的意义。方法 1 45例经病理证实不同胃黏膜病变 ,采用免疫组织化学法检测c -met基因表达 ,采用原位末端标记法(TUNEL)检测细胞凋亡。用Long -rank法检测胃癌生存率。结果 在浅表性胃炎 (CSG)、萎缩肠化生胃炎 (CAG +IM)、异型增生 (DYS)、早期GC和进展期GC中 ,c -met基因表达率分别为 2 4% ,5 1 % ,62 % ,67%和 68% ,CAG +IM、DYS、GC均显著高于CSG(P <0 0 5 )。凋亡指数 (AI)分别为 (4 5 5± 2 3 3 ) % ,(6 43± 5 60 ) % ,(6 45± 5 1 2 ) % ,(6 5 5± 4 80 ) %及 (8 84± 5 63 ) % ,进展期GC显著高于CSG(P <0 0 5 )。胃黏膜凋亡指数与c -met表达强度有密切相关 (P <0 0 5 )。c -met阳性表达与胃癌组织类型、浆膜浸润和淋巴结转移密切相关 ,而且BorrmannⅣ明显高于早期胃癌和BorrmannⅠ ,Ⅱ (P <0 0 5 )。c-met阳性表达胃癌患者生存率显著差于阴性表达者。结论 c -met基因表达与胃黏膜增殖和恶化有关 ,且与凋亡有相关性。c -met基因可能成为评估胃癌预后的一项新指标。
Objective To study the expression and relationship of c-met gene protein and apoptosis in gastric mucosal lesions, and to explore the significance of c-met expression in the prognosis of gastric cancer (GC). Methods Forty-five cases of pathologically confirmed gastric mucosal lesions were examined for c-met gene expression by immunohistochemistry and apoptosis was assessed by TUNEL. Long-rank method was used to detect the survival rate of gastric cancer. Results In superficial gastritis (CSG), atrophic intestinal gastroenteritis (CAG +IM), dysplasia (DYS), early GC, and advanced GC, the expression rate of c-met gene was 2 4% and 51 % respectively. , 62%, 67% and 68%, CAG + IM, DYS, GC were significantly higher than CSG (P <0 05). The apoptotic index (AI) was (4 5 5 ± 2 3 3) %, (6 43 ± 5 60) %, (6 45 ± 5 1 2) %, (6 5 5 ± 4 80) % and (8) respectively. 84 ± 5 63 ) %, advanced GC was significantly higher than CSG (P <0 05). The apoptotic index of gastric mucosa was closely related to the expression of c-met (P <0 05). The positive expression of c-met was closely related to histological type, plasma membrane infiltration and lymph node metastasis, and Borrmann IV was significantly higher than early gastric cancer and Borrmann I, II (P < 0.05). The survival rate of c-met positive gastric cancer patients was significantly lower than that of negative patients. Conclusion The expression of c-met gene is related to the proliferation and worsening of gastric mucosa, and it is related to apoptosis. The c-met gene may be a new indicator for evaluating the prognosis of gastric cancer.
