大剂量动态静脉尿路造影加延迟摄片在中、重度肾积水患者中的应用

目的探讨大剂量动态静脉尿路造影加延迟摄片在中、重度肾积水患者中的临床应用价值。方法对超声诊断单侧中、重度肾积水的132例患者行无压式大剂量静脉尿路造影,并在透视监视下实时点片,对显影欠佳者行延迟摄片,延迟时间1~12h。结果 132例患者中98例成功显示梗阻部位及梗阻以上扩张的肾盂肾盏及输尿管;27例患者浅淡显影但未能显示梗阻部位,7例未显影,对上述34例进一步行逆行造影明确梗阻部位。结论对超声诊断中、重度肾积水的患者直接行大剂量动态静脉尿路造影加延迟摄片不仅能提高对泌尿系梗阻诊断的准确率,还可以有效减少有创检查的必要性。 Objective To investigate the clinical value of high-dose dynamic intravenous urography plus delayed imaging in patients with moderate and severe hydronephrosis. Methods One hundred and thirty-two patients with unilateral hydronephrosis and severe hydronephrosis were treated with non-pressured high-dose intravenous urography and real-time fluoroscopy under fluoroscopy. Delayed radiographs were performed on those with poor development. The delay time was 1 ~ 12h. Results Of the 132 patients, 98 patients successfully showed the obstructive site and the renal pelvis and ureter with dilated obstruction. In 27 patients, the development was obscure but failed to show the obstruction site. Seven patients were undeveloped. The retrograde angiography was obstructed in 34 patients Site. Conclusion In the diagnosis of ultrasound, severe hydronephrosis in patients with direct high-dose intravenous urography and delayed radiography can not only improve the accuracy of diagnosis of urinary tract obstruction, but also can effectively reduce the need for invasive examination.
