目的:研究唇腭裂修复术对下颌骨形态和位置的影响。方法:选取21例非唇腭裂患者(非裂患者)作为A组(男10例,女11例,平均年龄13.10岁),19例单侧唇腭裂伴牙槽突裂患者作为B组(男11例,女8例,平均年龄11.57岁),17例双侧唇腭裂伴牙槽突裂患者作为C组(男10例,女7例,平均年龄12.50岁)。在正畸治疗前进行头影测量,分析下颌骨的形态和位置,使用SPSS17.0软件包对3组患者的测量数据进行独立样本t检验。结果:单侧唇腭裂患者与非裂患者在ANSMe、SNB、BaNPog和CoGo-MP存在显著差异(P<0.05),在CoGo、GoGn、CoGn、SGo和NMe也存在显著差异(P<0.01)。双侧唇腭裂患者与非裂患者在GoGn和SN-MP存在显著差异(P<0.05),在SNB和CoGo-MP也存在显著差异(P<0.01)。结论:唇腭裂修复术对下颌骨生长发育的影响主要表现在下颌骨矢状向位置偏后,下颌体长度减小,下颌支高度降低以及下颌骨向后、向下旋转。
Objective: To study the effect of cleft lip and palate repair on mandible shape and location. Methods: Twenty-one patients with non-cleft lip and palate (non-cleft palate) were selected as group A (10 males and 11 females, average age 13.10 years), and 19 patients with cleft lip and cleft palate (group B) (8 males and average age 11.57 years old). Seventeen patients with bilateral cleft lip and cleft palate with alveolar cleft were selected as group C (10 males and 7 females, average age, 12.50 years). Cephalometrical measurements were performed before orthodontic treatment. Morphology and location of the mandible were analyzed. Independent samples t-test was used to measure the data of 3 groups using SPSS17.0 software package. Results: There were significant differences in ANSMe, SNB, BaNPog and CoGo-MP between unilateral cleft palate and non-cleft palate (P <0.05). There was also significant difference between CoGo, GoGn, CoGn, SGo and NMe (P <0.01). There was a significant difference in GoGn and SN-MP between patients with bilateral cleft lip and palate and those with non-cleft palate (P <0.05). There was also significant difference between SNB and CoGo-MP (P <0.01). Conclusion: The effect of cleft lip and palate repair on the growth and development of the mandible is mainly manifested in the mandibular sagittal position, the mandibular body length decreases, mandibular height decreased and the mandible backward and downward rotation.
