新生儿缺氧缺血性心脏损害QT间期离散度及血清CK-MB的变化及预后分析

为探讨新生儿缺氧缺血心脏损害 (HIM)患儿 QT间期离散度 (QTcd)和血清肌酸激酶同功酶 B(CK- MB)的动态变化及对 HIM预后的影响。采用对 6 3例 HIM患儿发病 48h内和治疗 7d后 QTcd和 CK- MB进行检测 ,并与 30例无窒息新生儿对照比较。结果显示 HIM组与对照组比较 ,患儿发病 48h内 QTcd及 CK- MB均显著升高 (P<0 .0 1 )。经 7d治疗后 ,HIM患儿 QTcd及 CK- MB均显著下降 ,产后缺氧组较胎儿期缺氧缺血组下降更显著 (P<0 .0 5 ) ,且胎儿期缺氧缺血组并发症多 ,预后差 (P<0 .0 5 )。结论 QTcd及 CK- MB相结合可作为 HIM的特异性敏感指标 ,对判断 HIM预后有益。 To investigate the dynamic changes of QTcd and serum creatine kinase B (CK-MB) in neonates with hypoxic-ischemic heart damage (HIM) and their effect on the prognosis of HIM. QTcd and CK-MB were detected in 48 h and 7 d of the onset of HIM in 63 HIM children and compared with 30 infants without asphyxia. The results showed that compared with the control group, the incidence of QTcd and CK-MB in HIM group was significantly increased within 48 hours (P <0.01). After 7 days of treatment, the QTcd and CK-MB in HIM children were significantly decreased, the hypoxia group after childbirth than the fetal hypoxic-ischemic group decreased more significantly (P <0. 05), and fetal hypoxic-ischemic group complicated Disease, poor prognosis (P <0. 05). Conclusion The combination of QTcd and CK-MB can be used as a sensitive index of HIM, which is beneficial to the prognosis of HIM.
