目的探讨血浆脂蛋白相关磷脂酶A2(Lp-PLA2)在缺血性脑血管病中的临床意义。方法检测87例缺血性脑血管病患者(A组)和50例健康体检者(B组)的血浆Lp-PLA2、TC、HDL-C、LDL-C、脂蛋白a(LPa)和FBG水平。结果与B组相比,A组的血浆Lp-PLA2[(247.66±105.08)ng/ml vs.(103.16±20.90)ng/ml]、TC[(5.09±1.18)mmol/L vs.(4.43±0.59)mmol/L]、LDL-C[(3.72±0.88)mmol/L vs.(3.12±0.82)mmol/L]、LPa[(282.59±234.00)mg/L vs.(136.56±92.25)mg/L]和FBG[(6.04±1.51)mmol/L vs.(5.55±0.57)mmol/L]水平均较高(P<0.01),HDL-C[(1.08±0.29)mmol/L vs.(1.43±0.28)mmol/L]水平较低(P<0.01)。Lp-PLA2诊断缺血性脑血管病的敏感度、特异度、准确性、阳性预测值和阴性预测值分别为60.92%、76.00%、66.42%、81.54%和52.78%;血浆Lp-PLA2水平与缺血性脑血管病严重程度密切相关(P<0.01)。结论血浆Lp-PLA2是预测缺血性脑血管病风险的一个生物学标志物。
Objective To investigate the clinical significance of plasma lipoprotein associated phospholipase A2 (Lp-PLA2) in ischemic cerebrovascular disease. Methods Plasma levels of Lp-PLA2, TC, HDL-C, LDL-C, LPa and FBG were measured in 87 patients with ischemic cerebrovascular disease (group A) and 50 healthy subjects (group B) . Results Plasma levels of Lp-PLA2 [(247.66 ± 105.08) ng / ml vs. (103.16 ± 20.90) ng / ml], TC [(5.09 ± 1.18) mmol / L vs. (4.43 ± 0.59) mmol / L], LPa [(282.59 ± 234.00) mg / L vs. (136.56 ± 92.25) mg / (P <0.01). HDL-C [(1.08 ± 0.29) mmol / L vs. (1.43 ± 0.29) mmol / L vs ± 0.28) mmol / L] (P <0.01). The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of Lp-PLA2 in diagnosing ischemic cerebrovascular disease were 60.92%, 76.00%, 66.42%, 81.54% and 52.78% respectively. The levels of Lp-PLA2 The severity of ischemic cerebrovascular disease is closely related (P <0.01). Conclusions Plasma Lp-PLA2 is a biomarker for predicting the risk of ischemic cerebrovascular disease.
