目的:探讨尿微量白蛋白(尿m-ALB)、尿液N-乙酰-氨基葡萄糖酐酶(NAG)及血清胱抑素C(Cys C)在老年高血压及糖尿病患者早期肾损害诊断中的临床价值。方法:对我科2010年—2012年住院检测血清肌酐正常的老年原发性高血压患者(82例)、糖尿病患者(49例)、高血压合并糖尿病患者(52例)及健康对照者(56例),用全自动生化仪检测尿液NAG、尿肌酐(CREAU)、尿m-ALB、尿液β2微球蛋白(β2-MG)、血清胱抑素C、血β2微球蛋白(血β2MG)水平,并对检测结果进行统计学分析。结果:老年高血压组及高血压合并糖尿病组血清Cys C、尿NAG、尿m-ALB及尿m Alb/Cr、尿NAG/Cr水平均高于健康对照组(P<0.01);尿NAG、尿m-ALB在高血压合并糖尿病组、糖尿病组均高于高血压组(P<0.01)。结论:尿NAG、尿mALB和血清Cys C是诊断老年高血压病、糖尿病早期肾损害的敏感指标,糖尿病为老年高血压患者加重肾损害的重要因素之一,尿NAG、尿m-ALB与血清Cys C联合检测具有较高阳性检出率,对判断早期肾损害和评价疗效均具有重要的临床价值。
Objective: To investigate the diagnostic value of urinary albumin (urine m-ALB), urine N-acetylglucosamine (NAG) and serum cystatin C (Cys C) in the diagnosis of early renal damage in elderly patients with hypertension and diabetes mellitus Clinical Value. Methods: A total of 82 elderly patients with essential hypertension who had normal serum creatinine in our department from 2010 to 2012, 49 patients with diabetes mellitus, 52 patients with hypertension and 52 healthy controls were enrolled in this study. In this study, urinary NAG, creatinine (CREAU), urine m-ALB, β2 microglobulin (β2-MG), serum cystatin C, blood β2 microglobulin ) Level, and the test results were statistically analyzed. Results: Serum levels of Cys C, urine NAG, urine m-ALB, urine m Alb / Cr and urinary NAG / Cr levels were significantly higher in elderly hypertensive patients and hypertensive patients with diabetes mellitus than those in healthy controls (P <0.01) Urinary m-ALB in hypertensive diabetic group and diabetic group were higher than those in hypertension group (P <0.01). Conclusion: Urinary NAG, urinary mALB and serum Cys C are sensitive indicators of hypertension and early renal damage in diabetes. Diabetes mellitus is one of the most important factors that aggravate renal damage in elderly hypertensive patients. Urinary NAG, urinary m-ALB and serum Cys C combined detection has a higher positive detection rate, which has important clinical value in judging the early renal damage and evaluating the curative effect.
