目的:评估超声弹性成像在乳腺良恶性病变诊断和鉴别诊断中的价值。方法:使用超声弹性成像观察120例乳腺疾病患者的120个病灶,使用5分评分系统进行评估。结果:组织病理检查证实,120个乳腺病灶中良性病变71个,恶性病变49个。良性病变的超声弹性成像评分平均为(2.04±0.36)分,恶性病变平均为(3.06±0.94)分(P<0.001)。受试者工作特征(ROC)曲线下面积为0.822,弹性成像鉴别乳腺良恶性病变的最佳界值位于2~3分,诊断的灵敏度、特异度和准确率分别为69.39%、91.55%和82.50%。结论:超声弹性成像有助于鉴别乳腺良恶性病变,有望成为辅助诊断乳腺恶性肿瘤的新手段。
Objective: To evaluate the value of elastography in the diagnosis and differential diagnosis of benign and malignant breast lesions. Methods: Echographic elastography was used to observe 120 lesions in 120 patients with breast disease and evaluated using a 5-point scoring system. Results: Histopathological examination confirmed 71 benign lesions and 49 malignant lesions in 120 breast lesions. The average score of ultrasound elastography in benign lesions was (2.04 ± 0.36) points, and the average of malignant lesions was (3.06 ± 0.94) points (P <0.001). The area under the receiver operating characteristic (ROC) curve was 0.822. The best cutoff value of elastography in differentiating benign and malignant breast lesions was 2 to 3, and the diagnostic sensitivity, specificity and accuracy were 69.39%, 91.55% and 82.50 %. Conclusion: Elastography can help identify benign and malignant breast lesions and is expected to be a new method to diagnose breast malignant tumors.
