目的研究复合前列腺特异性抗原(c-PSA)及相关指标在鉴别诊断前列腺癌和良性前列腺增生中的应用价值。提高前列腺癌诊断特异性。方法分析研究60例男性,经B超、病理诊断证实18例为前列腺癌(PCa),27例为良性前列腺增生(BPH),25例为正常对照组。Bayer ACS:180化学发光法测得c-PSA、t-PSA,计算c-PSA/t-PSA比值(c/t)。结果t检验比较PCa和BPH组的c-PSA、t-PSA、C/T和年龄值,前两者P<0.001,后两者P>0.05。分别计算PCa和BPH组的c-PSA、t-PSA的直线相关系数r,PCa组r为0.9812,PCa组回归方程为c-PSA=0.9354×t-PSA-2.91,高度相关。BPH组r仅为0.4109。在100%敏感性时,c-PSA3.75μg/L、t-PSA4.0μg/L、c/t=0.65、c-PSA7.5μg/L+c/t0.65的特异性分别为42.3%、38.5%、36.5%、75.0%。结论前列腺癌c-PSA比BPH对t-PSA相关度高。在保持灵敏度100%时,若结合c-PSA7.5μg/L+c/t0.65两个指标,将明显提高特异性到75.0%。
Objective To study the value of combined prostate-specific antigen (c-PSA) and related indicators in the differential diagnosis of prostate cancer and benign prostatic hyperplasia. Improve the diagnostic specificity of prostate cancer. Methods Sixty male patients were diagnosed by B ultrasound. 18 cases were diagnosed as prostate cancer (PCa) by pathological diagnosis, 27 cases were benign prostatic hyperplasia (BPH) and 25 cases were normal control group. Bayer ACS: 180 chemiluminescence method measured c-PSA, t-PSA, calculated c-PSA / t-PSA ratio (c / t). Results t-test compared PCa and BPH group c-PSA, t-PSA, C / T and age values, the former two P <0.001, the latter two P> 0.05. The linear correlation coefficients of c-PSA and t-PSA in PCa and BPH groups were calculated respectively. The r of PCa group was 0.9812, and the regression equation of PCa group was c-PSA = 0.9354 × t-PSA-2.91. BPH group r only 0.4109. The specificity of c-PSA3.75μg / L, t-PSA4.0μg / L, c / t = 0.65, c-PSA7.5μg / L + c / t0.65 at the sensitivity of 100% were 42.3% 38.5%, 36.5%, 75.0%. Conclusion Prostate cancer c-PSA has a higher correlation with BPH than t-PSA. While maintaining the sensitivity of 100%, if combined with c-PSA7.5μg / L + c / t0.65 two indicators, will significantly improve the specificity to 75.0%.
