目的:探讨缺氧诱导因子-1α(HIF-1α)、血管内皮细胞生长因子(VEGF)、微血管密度(MVD)在不同宫颈病变组织中的表达特点及与临床病理特征的关系。方法:应用免疫组织化学SP法检测14例慢性宫颈炎、16例CINⅠ级、19例CINⅡ~Ⅲ级和65例宫颈鳞癌组织中HIF-1α和VEGF的表达情况,用CD34检测微血管密度。结果:HIF-1α在慢性宫颈炎、CINⅠ级、CINⅡ~Ⅲ级和宫颈鳞癌中的阳性率分别为0.00%、12.50%、31.58%和75.38%;VEGF在以上病变中的阳性率分别为0.00%、18.75%、36.84%和72.31%。HIF-1α和VEGF在宫颈鳞癌中的表达明显高于炎症组、CINⅠ级组和CINⅡ~Ⅲ级组,差异有统计学意义(P<0.05)。HIF-1α和VEGF的表达与患者临床分期、病理组织分型和淋巴结转移有关(P<0.05),与患者年龄无关。MVD和HIF-1α、VEGF的表达特点相同,在子宫颈鳞癌中HIF-1α、VEGF和MVD的表达水平呈正相关(P<0.01)。结论:HIF-1α和VEGF在子宫颈鳞癌中的高表达、MVD值的高水平可能在子宫颈鳞癌的发生、发展和转移中发挥重要作用,可以作为诊断和治疗的新靶点。
Objective: To investigate the expression of hypoxia inducible factor-1α (HIF-1α), vascular endothelial growth factor (VEGF) and microvessel density (MVD) in different cervical lesions and its relationship with clinicopathological features. Methods: Immunohistochemical SP method was used to detect the expression of HIF-1α and VEGF in 14 cases of chronic cervicitis, 16 cases of CINⅠ, 19 cases of CINⅡ ~ Ⅲ and 65 cases of cervical squamous cell carcinoma. The microvessel density was detected by CD34. Results: The positive rates of HIF-1α in chronic cervicitis, CINⅠ, CINⅡ ~ Ⅲ and cervical squamous cell carcinoma were 0.00%, 12.50%, 31.58% and 75.38% respectively. The positive rates of VEGF in the above lesions were 0.00 %, 18.75%, 36.84% and 72.31%. The expression of HIF-1α and VEGF in cervical squamous cell carcinoma was significantly higher than that in inflammatory group, CINⅠgroup and CINⅡ-Ⅲgroup (P <0.05). The expression of HIF-1α and VEGF were correlated with clinical stage, pathological type and lymph node metastasis (P <0.05), but not with patient’s age. The expression of MVD, HIF-1α and VEGF were the same, and there was a positive correlation between the expression of HIF-1α, VEGF and MVD in cervical squamous cell carcinoma (P <0.01). Conclusion: HIF-1α and VEGF are highly expressed in cervical squamous cell carcinoma. The high level of MVD may play an important role in the occurrence, development and metastasis of cervical squamous cell carcinoma, which may serve as a new target for diagnosis and treatment.
