回顾分析了34例垂体巨腺瘤(直径≥4cm)经颅显微手术治疗的临床资料。所有病例均经翼点入路(23例)、冠状切口入路(7例)、右额马蹄形切口入路(3例)或联合切口入路(1例),采用显微手术切除肿瘤,其中腺瘤全切除28例,大部分切除6例,无死亡病例。所有病例均行术后放射治疗(放射剂量30~50GY/3—5周)。复发2例,占5.9%。结果表明,经颅显微外科手术治疗垂体巨腺瘤较为安全,可明显改善视觉功能,术后并发症少,也是提高全切率、降低死亡率和复发率的重要方法。
Retrospective analysis of 34 cases of pituitary adenoma (diameter ≥ 4cm) transcranial microsurgery clinical data. All cases were treated by microsurgical resection of the tumor with pontine approach (n = 23), coronal incision (n = 7), right forearm horseshoe incision (n = 3) or combined incision approach Total adenoma removal in 28 cases, most of the resection in 6 cases, no deaths. All cases underwent postoperative radiotherapy (radiation dose 30 ~ 50GY / 3-5 weeks). Recurrence in 2 cases, accounting for 5.9%. The results show that transcranial microsurgery is a safe treatment for pituitary adenoma, which can significantly improve visual function and reduce the postoperative complications. It is also an important method to increase the rate of complete resection, reduce mortality and recurrence rate.
