目的观察铸造联冠把下颌阻生智齿与近中根管治疗后的下颌第二磨牙连在一起的修复效果。方法选择2006年5月—2008年5月下颌高位阻生智齿,近中下颌第二磨牙患牙体牙髓根尖周病变根管治疗后无症状的患者65例,设计下颌第二、三磨牙铸造联冠共同就位道,常规备牙取模灌模,制作下颌第二、三磨牙铸造联冠,试戴调颌粘接,随访观察1~2年。结果患者治疗后2年内每半年复诊1次,第1年复查结果:成功率98.5%;失败1例。第2年复查结果:成功率98.4%;失败1例为铸造联冠松动。结论下颌第二、三磨牙铸造联冠把下颌第二、三磨牙连为一体,形成1个功能单位,解决了第二磨牙和第三磨牙之间的食物嵌塞,利用下颌第三磨牙的牙周储备力,分散了下颌第二磨牙承受的颌力或咀嚼压力,避免了冠折根裂,提高了咀嚼效率,修复后效果良好。适用于存在拔牙禁忌证的中老年患者。
Objective To observe the healing effect of casting combined crown on mandibular second molars treated with mandibular impacted wisdom teeth and proximal root canal.
