目的 :为评价经食管心房调搏心脏负荷试验 (简称负荷试验 )前后 QT离散度 (QTd、QTcd)的变化及在缺血性心脏病诊断中的意义。方法 :对 19例负荷试验阳性者 (阳性组 )及 2 4例负荷试验阴性者 (阴性组 )前后各时段心电图 QTd、QTcd进行对比观察。结果 :试验前及试验后 4,6 ,8m in两组 QTd、QTcd变化差异均无显著性 (P均 >0 .0 5 ) ;两组比较试验后即刻、 2 min的 QTd、 QTcd阳性组较阴性组增大 (P均 <0 .0 1) ;阳性组负荷试验后即刻、 2 min时 QTd、 QTcd均较试验前显著延长 (P均 <0 .0 1) ,试验后 4,6 ,8min时的 QTd、QTcd与试验前比较差异均无显著性 (P >0 .0 5 ) ;阴性组试验后各时段与试验前比较差异均无显著性 (P >0 .0 5 )。结论 :经食管心房调搏心脏负荷试验后即刻、 2 min时 QTd、QTcd显著延长可作为辅助诊断冠心病的有效指标之一。
Purpose : To evaluate the changes of QT dispersion (QTd, QTcd) before and after transesophageal atrial pacing heart stress test (abbreviated as stress test) and its significance in the diagnosis of ischemic heart disease. Methods : Electrocardiogram QTd and QTcd were compared between 19 patients with positive stress test (positive group) and 24 patients with negative stress test (negative group) before and after. Results: There was no significant difference in QTd and QTcd between the two groups (P>0.05) before and after the trial, and the QTd and QTcd positive groups were immediately after the two groups compared with 2 min. The negative group was increased (P<0.01); QTd and QTcd were significantly prolonged (P<0.01) at 2 min immediately after the positive group stress test, and 4, 6, and 8 min after the test. At the time of QTd and QTcd, there was no significant difference between pre-test and QTd (P > 0.05); there was no significant difference between the negative group and the pre-test period (P > 0.05). Conclusion : The significant prolongation of QTd and QTcd immediately after transesophageal atrial pacing heart stress test can be used as one of the effective indexes in the diagnosis of coronary heart disease.
