目的探讨胆囊癌根治性胆囊切除术对胆囊癌患者营养与免疫水平的影响。方法序列研究27例(男8例,女19例)因患胆囊癌行根治性胆囊切除术的患者的营养和免疫水平平均年龄58.5岁分别于术前、后3d,1wk,2wk,3wk测定营养和兔疫指数.用生物化学的方法测定营养水平值:血清清蛋白、胆固醇、铁、转铁球蛋白、镁、锌、血清总铁结合量、兔疫水平值的测定项目有:T淋巴细胞亚群、免疫球蛋白、补体C3和C4值;血清白介素-2,可溶性白介素-2受体值结果术后3d,所有的营养指数均急剧下降(P<0.01)术后2wk~3wk,大部分营养指数恢复到术前水平,但是血清铁、转铁球蛋白和血铁总铁结合量在术后3wk仍未恢复到术前水平.术后2wk~3wk后,免疫指数已恢复到术前水平结论胆囊癌根治性切除术对患者营养和免疫水平的影响是暂时的和可逆转的,手术2wk后应补充足量的铁元素,同时要定期检查微量元素水平
Objective To investigate the effect of radical cholecystectomy for gallbladder carcinoma on the level of nutrition and immunity in gallbladder cancer patients. METHODS: The nutritional and immunological levels of 27 patients (8 males and 19 females) who underwent radical cholecystectomy for gallbladder cancer were studied. Mean age 58.5 years before and after 3d, 1wk, 2wk, 3wk Determination of nutrition and rabbit disease index. Determination of nutritional levels using biochemical methods: Determination of levels of serum albumin, cholesterol, iron, transferrin, magnesium, zinc, serum total iron binding, and rabbit disease levels: T lymphocyte subsets, immunoglobulin Protein, complement C3 and C4 values, serum interleukin-2, and soluble interleukin-2 receptor results All nutritional indices decreased sharply (P<0.01) after 3 days postoperatively, and most of the nutritional indices recovered after 2 weeks to 3 weeks. At the preoperative level, serum iron, transferrin, and total iron combined did not return to preoperative levels at 3 weeks after surgery. After 2wk~3wk of surgery, the immune index has returned to preoperative levels. Conclusion The radical resection of gallbladder cancer has a temporary and reversible effect on the level of nutrition and immunity of patients. After 2wk of surgery, a sufficient amount of iron should be added. To regularly check the level of trace elements
