目的:研究重庆地区青少年下颌第二磨牙(mandibular second molars,Msm)生长情况与个体年龄的相关性。方法:选取我院正畸科门诊的357例青少年病人,通过锥形束断层扫描(cone beam computed tomography,CBCT)视图精确测量Msm牙根长、牙冠长和宽,分别计算三者相关比值。对Msm相关长度指标与年龄的关系进行Pearson相关分析,根据相关性分析结果选取适宜参数拟合年龄推导的数学方程式,并对其年龄判别的准确性进行验证。结果:在各组别中Pearson相关性分析表明Msm的根长和年龄呈正相关(r_(男性)=0.659,P=0.000;r_(女性)=0.572,P=0.000,r_(总体)=0.608,P=0.000),根长/冠宽和年龄呈正相关(r_(男性)=0.580,P=0.000;r_(女性)=0.551,P=0.000,r_(总体)=0.543,P=0.000),冠长/根长和年龄呈负相关(r_(男性)=-0.497,P=0.000;r_(女性)=-0.448,P=0.000,r_(总体)=-0.470,P=0.000)。年龄(Y)和Msm根长(X_1)、根长/冠宽(X_2)、冠长/根长(X_3)的相关方程为:Y_(男性)=4.165+0.341X_1+2.151X_2+0.882X_3(P=0.000,R~2=0.547);Y_(女性)=5.057+0.255X_1+2.938X_2-0.160X_3(P=0.000,R~2=0.466);Y_(总体)=4.918+0.285X_1+2.521X_2+0.240X_3(P=0.000,R~2=0.499)。在男性、女性及总体样本中,该方法评估生理年龄的相对误差分别为5.00%、5.90%和5.46%。结论:重庆地区青少年Msm与年龄具有一定相关性,采用相关指标拟合数学方程式评判生理年龄为青少年生长发育水平评估方法的研究提供了新的思路。
Objective: To study the correlation between the growth of mandibular second molar (Msm) and individual age in adolescents in Chongqing. Methods: A total of 357 adolescents with orthodontic clinics in our hospital were selected to measure the root length, crown length and width of Msm accurately by the cone beam computed tomography (CBCT) view. Pearson correlation analysis was used to analyze the relationship between the correlation length of Msm and age. According to the results of correlation analysis, appropriate parameters were fit to age-derived mathematical equations and the accuracy of age discrimination was verified. Results: The Pearson correlation analysis showed that the root length and age of Msm were positively correlated (r_ (male) = 0.659, P = 0.000; r_ (women) = 0.572, P = 0.000 and r_ (overall) (R = (male) = 0.580, P = 0.000; r_ (female) = 0.551, P = 0.000, r_ (overall) = 0.543, P = 0.000) Length / root length was negatively correlated with age (r_ (male) = -0.497, P = 0.000; r_ (female) = -0.448, P = 0.000, r_ (overall) = -0.470, P = 0.000). The correlation equation between age (Y) and Msm root length (X_1), root length / crown width (X_2) and crown length / root length (X_3) is: Y_ (male) = 4.165 + 0.341X_1 + 2.151X_2 + 0.882X_3 Y = (total) = 4.918 + 0.285X_1 + 2.521X_2 (P = 0.000, R ~ 2 = 0.547) + 0.240X_3 (P = 0.000, R ~ 2 = 0.499). In men, women and the overall sample, the relative error of the method in assessing physiological age were 5.00%, 5.90% and 5.46%, respectively. Conclusion: There is a certain correlation between Msm in adolescent and age in Chongqing. It provides a new way to evaluate the relationship between maternal age and adolescents’ growth and development.
