目的:探讨CD10-MUM1+滤泡性淋巴瘤(FL)和CD10+MUM1-的FL中BCL2/IGH融合基因、BCL6分离基因和相应蛋白表达。方法:收集广西医科大学第一附属医院和广西民族医院2005年1月至2011年6月诊断为FL的患者54例的肿瘤组织,免疫组织化学方法检测BCL2蛋白和BCL6蛋白,荧光原位杂交技术(FISH)检测两组的BCL2/IGH融合基因、BCL6分离基因。结果:(1)BCL2蛋白表达,BCL2融合基因和BCL6分离基因在CD10+MUM1-的FL和CD10-MUM1+的FL中出现差异,差异有统计学意义。(2)BCL2蛋白表达和BCL2融合基因在CD10+MUM1-的FL和CD10-MUM1+的FL中呈负相关(分别为r=-0.337,P=0.013;r=-0.375,P=0.005)。BCL6分离基因在CD10-MUM1+FL和CD10+MUM1-FL中呈正相关(r=0.392,P=0.003)。结论:CD10+MUM1-的FL基本上是经典的FL,CD10-MUM1+的FL包含较多的高级别FL,与CD10+MUM1-的FL相比,少见BCL2蛋白表达和BCL2/IGH融合基因表达,可见BCL2基因扩增,常见BCL6分离基因。
Objective: To investigate the expression of BCL2 / IGH fusion gene, BCL6 gene and corresponding protein in FL of CD10-MUM1 + follicular lymphoma (FL) and CD10 + MUM1-. Methods: Tumor tissues of 54 patients diagnosed as FL from January 2005 to June 2011 were collected from the First Affiliated Hospital of Guangxi Medical University and Guangxi National Hospital. The BCL2 protein and BCL6 protein were detected by immunohistochemistry. Fluorescence in situ hybridization (FISH) BCL2 / IGH fusion gene and BCL6 gene were detected in both groups. Results: (1) BCL2 protein expression, BCL2 fusion gene and BCL6 gene segregation in FL and CD10-MUM1 + of CD10 + MUM1- FL were different, the difference was statistically significant. (2) BCL2 protein expression was negatively correlated with BCL2 fusion gene in FL for CD10 + MUM1- and FL for CD10-MUM1 + (r = -0.337, P = 0.013; r = -0.375, P = 0.005, respectively). The BCL6 gene was positively correlated with CD10-MUM1 + FL and CD10 + MUM1-FL (r = 0.392, P = 0.003). Conclusions: FL for CD10 + MUM1- is essentially a classical FL, FL for CD10-MUM1 + contains more high-grade FL, rare BCL2 protein expression and BCL2 / IGH fusion gene expression compared to CD10 + MUM1- Can be seen BCL2 gene amplification, common BCL6 isolated genes.
