目的观察原发性痛风性关节炎(GA)患者不同糖代谢状态下血清Nesfatin-1水平的变化。方法将研究对象分为GA组92例和正常(NC)组75名。GA组按血糖情况再分为GA-NGT亚组30例、GA-IGR亚组30例和新诊断GA-T2DM亚组32例。检测各组血糖、血脂、SUA、FIns、Nesfatin-1、HbA1c水平,稳态模型评估胰岛素抵抗指数(HOMA-IR)。结果 GA组Nefastin-1水平高于NC组[(6.00±0.86)vs(4.79±1.28)ng/ml](P<0.05)。GA-T2DM、GA-IGR亚组Nesfatin-1水平均高于GA-NGT亚组[(6.75±0.42)vs(5.88±0.67)vs(5.30±0.73)ng/ml](P<0.05)。多元逐步回归分析显示,Nesfatin-1与2hPG、SUA、FIns呈正相关(P<0.05)。结论 GA患者Nesfatin-1水平升高,Nesfatin-1可能参与了GA患者糖代谢调节。
Objective To observe the changes of serum Nesfatin-1 in patients with primary gouty arthritis (GA) under different glucose metabolism status. Methods The subjects were divided into GA group 92 cases and normal group (NC) 75 cases. GA group was further divided into GA-NGT subgroup 30 cases, GA-IGR subgroup 30 cases and newly diagnosed GA-T2DM subgroup 32 cases according to the blood glucose condition. The levels of blood glucose, blood lipids, SUA, FIns, Nesfatin-1 and HbA1c in each group were measured. The homeostasis model was used to evaluate the insulin resistance index (HOMA-IR). Results The level of Nefastin-1 in GA group was significantly higher than that in NC group [(6.00 ± 0.86) vs (4.79 ± 1.28) ng / ml] (P <0.05). The levels of Nesfatin-1 in GA-T2DM and GA-IGR subgroups were significantly higher than those in GA-NGT subgroups (6.75 ± 0.42 vs 5.88 ± 0.67 vs 5.30 ± 0.73 ng / ml, P <0.05). Multiple stepwise regression analysis showed that Nesfatin-1 was positively correlated with 2hPG, SUA and FIns (P <0.05). Conclusion Nesfatin-1 is elevated in patients with GA, and Nesfatin-1 may be involved in the regulation of glucose metabolism in patients with GA.
