目的研究探讨新生儿早发型败血症与母亲围产期高危因素的相关性。方法法选择我院新生儿科2013年9月~2015年8月确诊及临床诊断早发型败血症的新生儿为病例组,同期入住未并发败血症的新生儿为对照组。分析研究母亲围产期的7个高危因素与新生儿早发型败血症之间的相关性。结果单因素分析中母亲分娩时发热(T≥38℃)、母亲妊娠期感染、胎膜早破≥18小时、羊水粪污染和不明原因窒息五种因素P<0.05;不明原因早产和糖尿病母亲婴儿两种因素P>0.05;多因素Logistic回归分析中母亲分娩时发热(T≥38℃)的OR=0.659<1且95%的置信区间上限为0.963<1;其他四种因素的OR值均大于1。结论母亲妊娠期感染、胎膜早破≥18小时、羊水粪污染、不明原因窒息为新生儿早发型败血症的独立危险因素。
Objective To study the relationship between early neonatal sepsis and high risk factors of perinatal mothers. Method selection of neonatal pediatrics in our hospital from September 2013 to August 2015 diagnosed and clinically diagnosed as early-onset sepsis in neonates as case group, admitted to the same period without sepsis in neonates as control group. To analyze the correlation between maternal perinatal 7 risk factors and neonatal early-onset sepsis. Results In the univariate analysis, there were five factors (P <0.05) in mothers with fever during childbirth (T≥38 ℃), mothers during pregnancy, premature rupture of membranes (≥18 hours), amniotic fluid excrement contamination and unexplained asphyxia; unexplained preterm and diabetic mothers OR = 0.659 <1 and the upper limit of 95% confidence interval of 0.963 <1 in multivariate logistic regression analysis. The OR of the other four factors were both greater than 1. Conclusion Maternal pregnancy infection, premature rupture of membranes ≥ 18 hours, amniotic fluid fecal contamination, unexplained asphyxia neonatal early-onset sepsis independent risk factors.
