目的:探讨在正常及髌骨半脱位时股四头肌腱与股骨髁接触的变化。方法:对50侧膝关节的股四头肌腱内面及其与股骨髁的接触进行了观测。结果:股四头肌腱内面与股骨髁接触区呈近似三角形或椭圆形,其周围有滑膜附着,内表面长3.7±1.0(1.7~6.3)cm,宽2.2±0.5(0.4~3.2)cm。26%(13侧)标本髌上囊与膝关节腔不相通,腱股隔此囊相接触。屈膝超过90°时,腱股相接触并逐渐增大。髌骨半脱位时,整个股四头肌腱外移,腱股接触改变为股四头肌腱与股骨外髁接触增多而与股骨内髁接触减少。结论:股四头肌腱与股骨髁接触是髌股关节的重要组成部分,对维持该关节功能起重要作用。髌骨半脱位时,腱股接触出现错格现象,这可能是髌骨软化症的病理因素之一。
Objective: To investigate the changes of the tendon of the quadriceps tendon in contact with the femoral condyle during normal and patellar subluxation. METHODS: The medial aspect of the quadriceps tendon in 50 lateral knees and its contact with the femoral condyle were observed. Results: The contact area between the medial quadriceps tendon and the femoral condyle was approximately triangular or oval with synovial membrane around it. The inner surface was 3.7 ± 1.0 (1.7 ~ 6.3) cm in width and 2 in width. 2 ± 0.5 (0.4 ~ 3.2) cm. 26% (13 sides) specimens of the suprapatellar capsule and the knee cavity is not connected, tendon shares the capsule contact with each other. Bend more than 90 °, tendon contact and gradually increase. When the patellar subluxation, the entire quadriceps tendon relocated, tendon contact changes tendon quadriceps tendon contact with the lateral femoral condyle increased contact with the femoral condyles decreased. CONCLUSIONS: Touching the tendon of the quadriceps tendon with the femoral condyle is an important part of the patellofemoral joint and plays an important role in maintaining the function of the joint. Patellar subluxation, tendon contact appeared wrong phenomenon, which may be one of the pathological factors of chondromalacia patellae.
