目的探讨临床上联合应用经颅多普勒超声和颈动脉超声在诊断脑血管疾病中的价值和意义。方法将我院在2012年1月至2014年10月期间收治的126例缺血性脑血管疾病患者随机分为A组、B组和C组,每组各42例,分别行经颅多普勒超声、颈动脉超声和联合超声。比较3组诊断方法的灵敏度、特异性、阳性预测值、阴性预测值。检测3组阳性患者甘油三酯(TG)、胆固醇(TC)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)、载脂蛋白A-I(Apo A-I)和载脂蛋白B(Apo B)水平。结果 C组诊断方法的灵敏度、特异性、阳性预测值、阴性预测值均显著高于A组和B组,组间差异有显著性(P<0.05);C组阳性患者TG、TC、LDL和Apo B水平显著低于A组和B组,HDL和Apo A-I显著高于A组和B组,组间差异均有显著性(P<0.05)。结论经颅多普勒超声联合颈动脉超声诊断缺血性脑血管疾病可提高诊断准确率,具有广阔的临床应用价值。
Objective To investigate the value and significance of clinical application of transcranial Doppler ultrasound and carotid artery ultrasound in the diagnosis of cerebrovascular diseases. Methods 126 cases of ischemic cerebrovascular diseases admitted from January 2012 to October 2014 in our hospital were randomly divided into group A, group B and group C, with 42 cases in each group. Transcranial Doppler Ultrasound, carotid ultrasound and combined ultrasound. The sensitivity, specificity, positive predictive value and negative predictive value of the three diagnostic methods were compared. The triglyceride (TG), cholesterol (TC), low density lipoprotein (LDL), high density lipoprotein (HDL), apolipoprotein AI (Apo AI) and apolipoprotein B (Apo B) Level. Results The sensitivity, specificity, positive predictive value and negative predictive value of the diagnostic methods in group C were significantly higher than those in group A and group B (P <0.05). The levels of TG, TC, LDL and The level of Apo B was significantly lower than that of A and B groups, HDL and Apo AI were significantly higher than those of A and B groups (P <0.05). Conclusion Transcranial Doppler echocardiography combined with carotid ultrasonography in the diagnosis of ischemic cerebrovascular disease can improve the diagnostic accuracy and has a wide range of clinical value.
