目的 研究富于淋巴细胞性经典型霍奇金淋巴瘤免疫表型及鉴别诊断。方法 5 6例单凭形态学诊断的霍奇金淋巴瘤进行SP法免疫组化检测 ,根据WHO新分类进行重新分类。结果 47例被诊断为霍奇金淋巴瘤 ,其中结节性淋巴细胞为主型 2例 ,经典型霍奇金淋巴瘤 43例 ,未分类 2例。经典型霍奇金淋巴瘤中富于淋巴细胞性 3例 ,结节硬化性 17例 ,混合细胞性 2 1例 ,淋巴细胞消减性 2例。富于淋巴细胞性的瘤细胞呈CD3 0和CD15阳性、CD2 0和CD3阴性 ;背景细胞中小淋巴细胞CD3阳性较多 ,其中TIA -1阳性细胞明显增多 ,CD5 7阳性细胞稀少。结论 瘤细胞对CD3 0 ,CD15 ,CD2 0和CD3及背景细胞对CD2 0 ,CD3 ,CD5 7和TIA -1的免疫表型有助于富于淋巴细胞性经典型霍奇金淋巴瘤的诊断与鉴别诊断。
Objective To study the immunophenotypic and differential diagnosis of classical lymphocytic Hodgkin’s lymphoma. Methods Fifty-six Hodgkin’s lymphoma diagnosed by morphological examination was immunohistochemically detected by SP method and reclassified according to the new WHO classification. Results 47 cases were diagnosed as Hodgkin’s lymphoma, of which nodular lymphocytes were predominant in 2 cases, classic Hodgkin lymphoma in 43 cases and unclassified in 2 cases. Classical Hodgkin’s lymphoma in lymphocyte rich in 3 cases, 17 cases of tuberous sclerosis, mixed cell 21 cases, 2 cases of lymphocyte depletion. Lymphoma-rich tumor cells were positive for CD30 and CD15 but negative for CD20 and CD3. Small CD3 positive cells were found in the background cells, especially TIA-1 positive cells and rare CD5 7-positive cells. Conclusion The immunophenotypes of tumor cells against CD30, CD15, CD20 and CD3 and background cells on CD20, CD3, CD57 and TIA-1 contribute to the diagnosis and prognosis of lymphocytic classic Hodgkin’s lymphoma Differential diagnosis.
