目的探讨放疗前后脑内肿瘤的MRI变化,以客观评价放疗的疗效。方法对51例脑内肿瘤(10例松果体区肿瘤,19例脑转移瘤和22例胶质瘤)放疗前后进行MR平扫和增强扫描,比较放疗前后肿瘤大小、占位效应、瘤周水肿及肿瘤内部改变。结果51例肿瘤放疗后症状改善42例(占82.5%),9例(17.6%)症状加重,而MRI示肿瘤缩小有37例(72.5%),其中6例完全消失,9例肿瘤大小不变,5例增大;占位效应缩小或消失39例(占76.5%);瘤周水肿缩小或消失有40例(78.4%)。4例临床症状加重而MRI表现为放射性脑病。结论MRI为一种能客观反映放疗前后脑肿瘤变化的无创性的敏感方法。
Objective To investigate the MRI changes of brain tumors before and after radiotherapy to evaluate the curative effect of radiotherapy objectively. Methods 51 cases of intracranial tumors (10 pineal tumors, 19 brain metastases and 22 gliomas) were scanned by MRI before and after radiotherapy. The tumor size, mass effect, tumor size Edema and tumor internal changes. Results Twenty-one patients (82.5%) had symptoms improvement after radiotherapy in 51 patients, and 9 patients (17.6%) had symptoms aggravated. MRI showed 37 patients (72.5%) with tumor shrinkage, of which 6 patients disappeared completely, 9 cases of tumor size unchanged, 5 cases increased; space effect reduced or disappeared in 39 cases (76.5%); peritumoral edema shrink or disappear in 40 cases (78.4%). 4 cases of clinical symptoms and MRI manifestations of radiation encephalopathy. Conclusion MRI is a noninvasive and sensitive method that can objectively reflect the changes of brain tumors before and after radiotherapy.
