目的观察微波凝固疗法在癌性下呼吸道阻塞治疗中的应用价值。方法对12例癌性下呼吸道阻塞患者进行经电子支气管镜微波热凝固治疗,观察其疗效及并发症的发生率。结果12例患者均在清醒状态下良好地接受了微波凝固治疗,微波凝固治疗后气道直径有所扩大(P<0.001),第1s用力呼气容量(FEV1)增加(P<0.001);术中均有少量出血,经稀释肾上腺素(肾上腺素1ml+NS10ml)3ml喷洒止血后,出血停止;1例术后出现急性左心衰(合并有冠心病);全部病例术后无发热症状出现。结论大功率微波热凝固疗法对癌性下呼吸道阻塞的治疗安全、有效,而且患者在治疗全过程中保持清醒状态。
Objective To observe the value of microwave coagulation therapy in the treatment of lower respiratory tract obstruction of cancer. Methods Twelve patients with obstructive lower respiratory tract obstruction were treated with microwave thermocoagulation by electronic bronchoscopy. The therapeutic effect and the incidence of complications were observed. Results All 12 patients received microwave coagulation therapy well under awake condition. The diameter of airway was enlarged after microwave coagulation therapy (P <0.001), forced expiratory volume (FEV1) increased at 1 s (P <0.001) In a small amount of bleeding, the diluted epinephrine (epinephrine 1ml + NS10ml) 3ml spray stop bleeding, stop bleeding; 1 case of acute left heart failure (with coronary heart disease); all cases without postoperative fever symptoms. Conclusion High-power microwave thermocoagulation therapy is safe and effective for the treatment of cancerous lower respiratory tract obstruction, and the patients remain awake during the whole course of treatment.
