目的比较布地奈德与地塞米松治疗儿童急性喉炎的疗效及对其免疫功能的影响。方法收集2014年1月至2015年12月湖北省妇幼保健院收治的急性喉炎患儿,分为普米克令舒雾化吸入组(观察组)和地塞米松组(对照组),比较两组患儿疗效、不良反应及治疗前后炎症因子和T淋巴细胞亚群的差异。结果观察组患儿治愈率显著高于对照组(P=0.004)。两组患儿治疗后呼吸困难、声嘶、犬吠样咳嗽和喉鸣评分等均较治疗前显著降低(均P<0.05),观察组患儿治疗后呼吸困难、声嘶、犬吠样咳嗽和喉鸣评分均显著低于对照组(均P<0.05)。观察组患儿呼吸困难、声嘶、犬吠样咳嗽和喉鸣消失时间均显著低于对照组(均P<0.05)。两组患儿治疗前高敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、白介素(IL)-6、IL-10、白细胞(WBC)、淋巴细胞(Lym)、CD3~+、CD4~+和CD8~+T淋巴细胞差异均无统计学意义(均P>0.05),治疗后hs-CRP、TNF-α、IL-6和WBC均较治疗前显著降低(均P<0.05),而Lym、CD3~+、CD4~+和CD8~+T淋巴细胞较治疗前差异均无统计学意义(均P>0.05);观察组患儿治疗后hs-CRP、TNF-α和IL-6均显著高于对照组(均P<0.05),而WBC显著低于对照组(P<0.05)。结论布地奈德治疗儿童急性喉炎疗效显著高于地塞米松,且对机体炎症反应及免疫功能影响较少。
Objective To compare the curative effect of budesonide and dexamethasone on acute laryngitis in children and its effect on immune function. Methods The children with acute laryngitis admitted to Hubei MCH Hospital from January 2014 to December 2015 were divided into two groups: imipenem inhalation group (observation group) and dexamethasone group (control group) The curative effect, adverse reaction and the differences of inflammatory cytokines and T lymphocyte subsets in children before and after treatment. Results The cure rate of observation group was significantly higher than that of control group (P = 0.004). After treatment, dyspnea, hoarseness, bark-like cough and laryngeal score in both groups were significantly lower than those before treatment (all P <0.05). After treatment, dyspnea, hoarseness, bark-like cough and throat Ming scores were significantly lower than the control group (P <0.05). The dyspnea, hoarseness, bark-like cough and disappearance of laryngeal mucosa in observation group were significantly lower than those in control group (all P <0.05). The levels of hs-CRP, TNF-α, IL-6, IL-10, WBC, Lym, CD3 (P> 0.05). After treatment, the levels of hs-CRP, TNF-α, IL-6 and WBC were significantly lower than those before treatment <0.05). There was no significant difference in Lym, CD3 ~ +, CD4 ~ + and CD8 ~ + T lymphocytes before treatment (all P> 0.05). The levels of hs-CRP, And IL-6 were significantly higher than the control group (all P <0.05), while WBC was significantly lower than the control group (P <0.05). Conclusion Budesonide treatment of children with acute laryngitis was significantly higher than dexamethasone, and the impact on the body’s inflammatory response and immune function less.
