目的:探讨和评价尿毒症合并慢性心力衰竭患者血液透析治疗中综合护理干预策略实施及效果。方法:选取我院血透室2016年01月~2020年01月间于实施维持性血液透析治疗的合并慢性心力衰竭的尿毒症患者84例,随机分为对照组(n=42)和观察组(n=42)。对照组接受常规血液透析常规护理及健康宣教,观察组接受控盐、控水、体重管理等容量管理行为能力,以及饮食、运动及心理自我调整管理综合干预。干预12周,对比两组干预前后控盐/控水/饮食容量管理情况、体质量指数(BMI)、心功能改善(NYHA心功能分级)、焦虑/抑郁情绪(SAS/SDS)调整及生活质量评价(QOL)。结果:干预前,两组控盐/控水/饮食容量管理行为、BMI、NYHA、SAS/SDS及QOL比较均无明显差异(P>0.05);干预后,观察组控盐/控水/饮食容量管理行为得分明显优于对照组(P<0.05);观察组BMI控制达标率及NYHA心功能分级Ⅱ级率明显高于对照组(P<0.05);观察组QOL各领域提升幅度明显高于对照组(P<0.05);观察组SAS/SDS自评得分明显低于对照组(P<0.05)。结论:尿毒症合并慢性心力衰竭患者血液透析治疗中实施综合护理干预,患者容量管理行为能力明显增强,体质量及心功能得到明显改善,焦虑抑郁情绪明显好转,患者生存质量明显提升。“,”Objective:To explore and evaluate the implementation and effect of comprehensive nursing intervention strategies in hemodialysis treatment for patients with uremia and chronic heart failure. Methods:A total of 84 uremic patients with chronic heart failure who were treated with maintenance hemodialysis in the hemodialysis room of our hospital between January 2016 and January 2020 were selected and randomly divided into a control group (n=42) and an observation group ( n=42). The control group received routine hemodialysis routine care and health education, and the observation group received volume management behaviors such as salt control, water control, and weight management, as well as comprehensive interventions in diet, exercise, and psychological self-adjustment management. Intervention for 12 weeks, comparing the salt control/water control/diet volume management, body mass index (BMI), improvement of cardiac function (NYHA cardiac function classification), anxiety/depressive mood (SAS/SDS) adjustment and quality of life before and after the intervention between the two groups Evaluation (QOL). Results:Before the intervention, there was no significant difference in the salt control/water control/diet volume management behavior, BMI, NYHA, SAS/SDS and QOL between the two groups (P>0.05); after the intervention, the observation group salt control/water control/diet volume The management behavior score was significantly better than that of the control group (P<0.05); the rate of BMI control compliance and NYHA heart function grading Ⅱ in the observation group was significantly higher than that of the control group (P<0.05); the improvement in QOL areas of the observation group was significantly higher than that of the control Group (P<0.05); The SAS/SDS self-evaluation score of the observation group was significantly lower than that of the control group (P<0.05). Conclusion:Comprehensive nursing intervention is implemented in the hemodialysis treatment of patients with uremia and chronic heart failure. The capacity management ability of the patients is significantly enhanced, the body weight and heart function are significantly improved, the anxiety and depression mood is significantly improved, and the quality of life of the patients is significantly improved.
