目的探寻贮存式自身输血在异基因造血干细胞移植供者中的应用效果。方法回顾性分析2013-2016年本院进行贮存式自身输血的127例异基因造血干细胞移植供者的造血干细胞采集及自体血回输情况。结果 127例供者共采集自体血70 550 m L,仅2例出现轻度献血反应,不良反应发生率为1.6%(2/127);采集骨髓血100 159m L。117例供者骨髓血采集前WBC(27.02±8.21)×1010/L、Plt(213.19±62.42)×109/L、RBC(4.31±0.44)×1012/L、Hb(131.53±14.53)g/L,自体血回输后1d WBC(35.85±9.08)×1010/L、Plt(188.34±50.24)×109/L、RBC(4.02±0.41)×1012/L、Hb(122.60±13.48)g/L。自体血回输后1 d造血干细胞供者WBC明显高于采髓前(P<0.05),Plt、RBC、Hb明显减低(P<0.05)。结论贮存式自身输血能节约血液资源,防止输血相关疾病发生,可应用于造血干细胞供者。
Objective To explore the application of self-contained blood transfusion in allogeneic hematopoietic stem cell transplantation. Methods The data of hematopoietic stem cell collection and autologous blood transfusion in 127 allogeneic hematopoietic stem cell transplantation patients undergoing self-transfusion in our hospital from 2013 to 2016 were retrospectively analyzed. Results A total of 127 donors received 70 550 m L of autologous blood. Only 2 patients showed mild blood donation reaction. The incidence of adverse reactions was 1.6% (2/127). 117 cases of donor bone marrow before WBC acquisition (27.02 ± 8.21) × 1010 / L, Plt (213.19 ± 62.42) × 109 / L, RBC (4.31 ± 0.44) × 1012 / L, Hb (131.53 ± 14.53) g / L , WBC (35.85 ± 9.08) × 1010 / L, Plt (188.34 ± 50.24) × 109 / L, RBC (4.02 ± 0.41) × 1012 / L and Hb (122.60 ± 13.48) g / L on the 1st day after autologous blood transfusion. The WBC of donor hematopoietic stem cells on day 1 after autologous blood transfusion was significantly higher than that of premedication (P <0.05), and Plt, RBC and Hb were significantly decreased (P <0.05). Conclusion The self-contained blood transfusion can save blood resources and prevent the occurrence of blood transfusion-related diseases, which can be applied to hematopoietic stem cell donors.
