目的:探讨食管鳞状细胞癌热休克蛋白27、70(HSP27、70)表达及临床意义。方法:应用SP免疫组化方法,检测101例食管鳞状细胞癌组织中HSP27、70的表达。根据临床病理资料和随访结果,对比分析HSP27和HSP70在临床病理特征相同、但术后生存时间明显不同的两组病例(长期生存组38例:术后生存5年以上;短期生存组63例:术后1年内死亡)中的表达情况,判定HSP27、70的表达与食管鳞状细胞癌临床病理特征及预后的关系。结果:HSP27在101例食管鳞状细胞癌中总的阳性表达率为88.1%(89/101),其中长期生存组总的阳性表达率为94.7%,强阳性表达率为81.6%,均明显高于短期生存组(84.1%和54.0%,P<0.05)。HSP70在101例中总的阳性表达率为96.0%(97/101),其中长期生存组总的阳性表达率和强阳性表达率分别为97.4%和68.4%,短期生存组分别为95.2%和76.2%,字2检验显示两组无显著性差异。HSP27和HSP70的表达与临床病理特征不相关(P>0.05)。Logistic回归分析显示HSP27的表达与食管鳞状细胞癌预后有关(P<0.01),而HSP70的表达与食管鳞状细胞癌预后无关(P>0.05)。结论:HSP27是食管鳞状细胞癌预后的独立因素,可作为判断食管鳞状细胞癌预后的重要生物指标之一。HSP70的表达与食管鳞状细胞癌预后无关。
Objective: To investigate the expression of heat shock protein 27, 70 (HSP27, 70) in esophageal squamous cell carcinoma and its clinical significance. Methods: SP immunohistochemical method was used to detect the expression of HSP27 and 70 in 101 cases of esophageal squamous cell carcinoma. According to clinicopathological data and follow-up results, HSP27 and HSP70 were compared in two groups of patients with the same clinicopathological features but different postoperative survival time (38 in the long-term survival group: 5 years in the postoperative survival group, 63 in the short-term survival group: 1 year after operation), and to determine the relationship between the expression of HSP27, 70 and clinicopathological features and prognosis of esophageal squamous cell carcinoma. Results: The total positive expression rate of HSP27 in 101 cases of esophageal squamous cell carcinoma was 88.1% (89/101). The total positive expression rate of HSP27 in long-term survival group was 94.7% and the strong positive expression rate was 81.6% In the short-term survival group (84.1% and 54.0%, P <0.05). The total positive expression rate of HSP70 was 96.0% (97/101) in 101 cases, of which the total positive expression rate and strong positive expression rate were 97.4% and 68.4% in long-term survival group and 95.2% and 76.2% in short-term survival group respectively %, Word 2 test showed no significant difference between the two groups. The expression of HSP27 and HSP70 was not correlated with clinicopathological features (P> 0.05). Logistic regression analysis showed that the expression of HSP27 was associated with the prognosis of esophageal squamous cell carcinoma (P <0.01), while the expression of HSP70 had no correlation with the prognosis of esophageal squamous cell carcinoma (P> 0.05). Conclusion: HSP27 is an independent prognostic factor for esophageal squamous cell carcinoma and may be used as one of the important biomarkers for predicting the prognosis of esophageal squamous cell carcinoma. The expression of HSP70 has nothing to do with the prognosis of esophageal squamous cell carcinoma.
