目的探讨超声下不同孕周胎儿主要生长参数正常值范围及其生长曲线分析。方法随机选取2014年12月-2015年12月在该院正常足月单胎分娩的产妇1 200例,回顾其应用超声检测的孕17~(+2)周~41~(+5)周共2 711次的主要生长参数:双顶径、头围、腹围和股骨长等,对各项指标与孕周进行相关性分析,以便得出主要生长参数的参考值范围,从而做出生长曲线,并将10年前的研究数据作为对照组。结果与对照组比较,孕27周以后双顶径、腹围和股骨长均呈增高趋势,两者差异有统计学意义(P<0.05);孕37周以后头围基本呈增高趋势,与对照组比较,差异有统计学意义(P<0.05)。对各项指标与孕周进行相关性分析,双顶径、头围、腹围及股骨长与孕周的相关系数r分别为0.968、0.972、0.973、0.979。结论胎儿的双顶径、头围、腹围和股骨长与孕周密切相关,且呈显著正相关,为制定出适合我国胎儿的生长参数标准,有必要更新生长参数正常值范围以及生长曲线。
Objective To investigate the normal range of growth parameters and its growth curve of fetus at different gestational weeks under ultrasound. Methods Twelve hundred and twenty maternal mothers undergoing normal full-term single-child delivery from December 2014 to December 2015 were retrospectively reviewed for gestational age from 17 weeks (+2 weeks) to 41 weeks (+5 weeks) 2 711 times the main growth parameters: biparietal diameter, head circumference, abdominal circumference and femur length, the correlation between indicators and gestational age in order to draw the main growth parameters of the reference value range, so as to make the growth curve , And 10 years ago, the research data as a control group. Results Compared with the control group, the biparietal diameter, abdominal circumference and femur length increased after 27 weeks of gestation, with a significant difference between the two groups (P <0.05). After 37 weeks of gestation, the head circumference was basically the same as that of the control The difference was statistically significant (P <0.05). Correlation analysis of each index and gestational age, biparietal diameter, head circumference, abdominal circumference and femur length and gestational age r correlation coefficient were 0.968,0.972,0.973,0.979. Conclusion Fetal biparietal diameter, head circumference, abdominal circumference and femur length are closely related to gestational age, and there is a significant positive correlation. It is necessary to update the normal range of growth parameters and growth curve in order to develop a suitable growth parameter standard for fetus in China.
