目的对喉癌组织中MMP-2、TIMP-2的表达与瘤内微血管密度(microvessel density,MVD)的相关性及预后意义进行探讨。方法对37例喉鳞癌患者的组织切片进行MMP-2、TIMP-2及CD34的免疫组化检测,用计算机图像分析系统对组织切片结果进行测定。结果淋巴结转移组MMP-2表达水平比非淋巴结转移组明显升高(P<0.01),而TIMP-2的表达两组之间没有统计学差异(P>0.05)。MVD均值转移组比非转移组明显高(P<0.01),MVD值与MMP-2表达水平之间呈线性相关(P<0.05)。并发现高MVD值组MMP-2的表达明显高于低MVD值组(P<0.05),高MMP-2表达组无瘤生存期分别比低MMP-2表达组明显缩短(P<0.01)。结论MMP-2可能在喉癌侵袭、转移和血管生成中承担着一个重要角色,并且MMP-2的表达可能促进了喉癌的淋巴播散能力,有望成为喉癌患者的一个预后目标。
Objective To investigate the correlation between the expression of MMP-2 and TIMP-2 and the microvessel density (MVD) in laryngeal carcinoma and its prognostic significance. Methods Tissue sections of 37 patients with laryngeal squamous cell carcinoma were detected by immunohistochemistry and MMP-2, TIMP-2 and CD34 were detected by computer image analysis system. Results The expression of MMP-2 in lymph node metastasis group was significantly higher than that in non-lymph node metastasis group (P <0.01), while the expression of TIMP-2 was not statistically different between the two groups (P> 0.05). The MVD mean metastasis group was significantly higher than the non-metastasis group (P <0.01), and the MVD value was positively correlated with the MMP-2 expression level (P <0.05). The results showed that the expression of MMP-2 in high MVD group was significantly higher than that in low MVD group (P <0.05), and the tumor-free survival in high MMP-2 group was significantly shorter than that in low MMP-2 group (P <0.01). Conclusions MMP-2 may play an important role in the invasion, metastasis and angiogenesis of laryngeal carcinoma, and the expression of MMP-2 may promote the lymphatic spread ability of laryngeal carcinoma and is expected to become a prognostic target of laryngeal cancer.
