目的评价实时双幅灰阶超声造影对肝胆管细胞癌的诊断与鉴别诊断价值。方法采用超声造影剂SonoVue对20例原发性肝细胞癌与12例胆管细胞癌患者行实时双幅灰阶超声造影检查。所有病例均经超声引导下穿刺活检或(和)手术病理证实。结果肝细胞癌组20例全部呈典型的“快进快退”型超声造影表现。胆管细胞癌组5例表现为早期动脉相呈边框样或中间少许增强,并迅速消退,呈“少进快退”型;另7例于整个造影过程中病灶均未见强化,呈“造影剂充盈缺损”型。结论实时双幅灰阶超声造影能为肝胆管细胞癌的诊断提供更多信息,弥补普通灰阶超声与彩色多普勒超声的不足。
Objective To evaluate the value of real-time double-graded contrast echocardiography in the diagnosis and differential diagnosis of hepatocellular cholangiocarcinoma. Methods SonoVue ultrasonic contrast agent in 20 cases of primary hepatocellular carcinoma and 12 cases of cholangiocarcinoma patients with real-time double gray-scale contrast echocardiography. All cases were confirmed by ultrasound-guided biopsy or (and) surgical pathology. Results All 20 cases of hepatocellular carcinoma showed typical “fast forward and reverberant” type of contrast-enhanced ultrasound. 5 cases of cholangiocarcinoma showed early arterial phase as a border-like or slightly enhanced in the middle, and quickly subsided, showing “less into reentrant” type; the other 7 cases in the whole process of imaging lesions were not enhanced, was “contrast agent Filling defect ”type. Conclusion Real-time double-grayscale contrast-enhanced ultrasound can provide more information for the diagnosis of hepatocellular carcinoma and make up for the deficiencies of common gray-scale ultrasound and color Doppler ultrasound.
